ASBESTOS SURVEY FOR BUILDING 732
DEMOLISH BUILDING
During April 2009, Mr. David Murray with 775 CES/CEPM requested an asbestos survey of Building 732 to demolish the building. The scope of this project is as follows:
Work Location: Building 732 Work Order: #48621
Statement of Work:
• Demolish the building.
• This inspection report is project specific and is not a valid report for any other project(s) associated with Building 732.
The information provided for this survey is based on samples collected by Nickolas King (Utah Certified Asbestos Inspector ASB-#2257). Samples for this survey were collected on 9 April 2009. The Science and Engineering laboratory, utilizing EPA Method 600R-93-116, analyzed all samples, which is PLM similar to NIOSH Manual of Analytical Methods 9002. Civil Engineering personnel also reviewed all available asbestos survey databases of suspect asbestos containing building materials (ACM) that could potentially be encountered in this area. The information gathered from all current and previous surveys is shown below by homogenous area.
Footages are based upon the scope of work provided by the requestor.
Batt insulation white/yellow (10,000’ sq) was sampled from the northwest corner, from the northeast corner, and from the center of the south side. These samples were analyzed as non-asbestos containing material (GM093336, GM093337, and GM093338).
All Hard Thermal System Insulation encountered above ceilings, behind walls, in wall chases, in attic spaces, and in mechanical rooms is assumed to be asbestos. This includes all insulation on pipe runs, pipe saddles, tanks, and pipe fittings.
Remember that the locations of these building materials (homogenous areas) indicate where these materials were found, but do not necessarily mean that these are the only locations where these materials may be encountered during the project. When any new suspect ACM is encountered, stop work immediately and contact either 75 CES/CEEV or SGPB to identify any new suspect building materials. As per state and federal regulations, if the scope of work or the work location changes, then a new asbestos inspection and asbestos survey report will be required.

MEMORANDUM FOR: 775 CES/CEPM (Mr. David Murray)
FROM: 75th CES/CEOHA
SUBJECT: Lead Based Paint (LBP) Survey, Building 732 (WO #48621). Demolish the building.
1. On 9 April 2009, Nickolas King (Utah Certified Lead Inspector #PB-1244) tested the painted surfaces found in Building 732 that would be impacted by demolishing the building. Building 732 was constructed in 1995. A Niton XL3t 300 XRF Analyzer (Serial # 30660) was used to test the painted surfaces of these components. Lead based paint was non-detected. Please refer to specific results of our survey; see attachment 1.
2. Limited lead-based paint identification has been completed for Building 732.
It is possible that lead based paint could be encountered during any renovation project.
Additional identification for lead based paint on other building components may be required. The facility manager should ensure that these components are surveyed for lead as well, to meet OSHA Standard 29 CFR 1926.62 construction requirements for lead.
3. The U.S. Department of Housing and Urban Development defines Lead-based paint as any paint, vanish, stain, or other applied coating that has 1mg/cm2 as measured by an X-ray Fluorescence (XRF) Analyzer or laboratory analysis or 0.5 percent by weight (5,000ug/g by dry weight) by laboratory analysis, or more of lead. All other components tested were less than 1 mg/cm2.
4. Any effort to disturb lead paint can create lead dust. Please ensure that appropriate abatement, cleanup, and disposal will be accomplished and that the appropriate safety measures are taken IAW 29 CFR 1926.62. If you have any questions please feel free to call Troye Davis at 586-7094.

MEMORANDUM FOR: 775 CES/CEPM (Mr. David Murray)
FROM: 75th CES/CEOHA
SUBJECT: Lead Based Paint (LBP) Survey, Building 1532 (WO #48621). Demolish the building.
1. On 29 April 2009, Nickolas King (Utah Certified Lead Inspector #PB-1244) tested the painted surfaces found in Building 1532 that would be impacted by demolishing the building. Building 1532 was constructed in 1988. A Niton XL3t 300 XRF Analyzer (Serial # 30660) was used to test the painted surfaces of these components. Lead based paint was non-detected. Please refer to specific results of our survey; see attachment 1.
2. Limited lead-based paint identification has been completed for Building 1532.

PART 3 EXECUTION
3.1 GENERAL REQUIREMENTS
The Contractor is responsible for quality control and shall establish and maintain an effective quality control system in compliance with the Contract Clause titled "Inspection of Construction." The quality control system shall consist of plans, procedures, and organization necessary to produce an end product which complies with the contract requirements. The system shall cover all construction operations, both onsite and offsite, and shall be keyed to the proposed construction sequence. The project superintendent will be held responsible for the quality of work on the job and is subject to removal by the Contracting Officer for non-compliance with quality requirements specified in the contract. The project superintendent in this context shall mean the individual with the responsibility for the overall management of the project including quality and production.
3.2 DESIGN QUALITY CONTROL (DQC)
The Contractor’s DQC Plan shall provide and maintain an effective quality control program which will assure that all services required by this design-build contract are performed and provided in a manner that meets professional architectural and engineering quality standards. The responsibility for checking and coordination of all design documents cannot be overemphasized.
The Contractor is responsible for producing complete, competent, properly coordinated, and thoroughly checked design documents within agreed schedules.
The Design Quality Control process for all Structural and Blast related design shall, as a minimum, include the requirement that each page of hand calculations or spreadsheet type printouts bear the legible name of the responsible engineer along with the most recent date of calculation. Each of these sheets shall also bear the handwritten initials of the assigned reviewer, as designated in the Government approved Design Quality Control Plan, along with the date of that review. Furthermore, all analysis and/or design computer run programs shall, likewise as a minimum, identify the responsible engineer and date of execution, however the reviewer need only initial and date those pages containing the input parameters and those containing any relevant controlling results
01 45 04.00 10-4
3.2.1 Design Quality Control Plan. The Contractor will be expected to follow the approved DQC plan throughout the course of the project to assure a quality end product. Should future events dictate revisions to the approved DQC Plan, the Contractor shall notify the Contracting Officer in writing and submit the revised plan for approval. The plan shall be prepared in 8-1/2” x 11” format. Any charts or schedules included in the plan that are larger than this size shall be folded to conform to an 8- 1/2” x 11” format. The plan shall include a title page and table of contents. The following elements shall be included in the DQC Plan:
a. Management Approach. Define the specific management methodology to be followed during the course of the contract including such aspects as design coordination procedures, quality control, communications, managerial continuity, flexibility and the following:
1. Competent, independent reviewers identified in the DQC Plan shall technically review all documents. The same element that produced the product shall not perform the independent technical review
(ITR).
2. The following elements shall affix a personal stamp to each drawing and specification section verifying their review and general compliance with the contract documents, as well as compliance as indicated:
--Design discipline – Compliance to the RFP --Independent Technical Reviewer – Compliance to the RFP
--CQC Staff member for each discipline identified in paragraph 3.5.3 CQC Personnel – Constructability and Operability.
--Prime and Key Subcontractors – Constructability and Operability
3. The contractor shall correct errors and deficiencies in the design documents prior to submitting them to the Government.
b. Management Structure. Delineate the organizational composition of the firm to clearly show the interrelationship of the management and design team components including all consultants. Include an organization chart to identify the key design and review team members showing their specific responsibilities.
c. Planning and Scheduling. The contractor shall include the design schedule, showing the sequence of events involved in carrying out the project tasks within the specific contract period. This should be at a detailed level of scheduling sufficient to identify all major tasks including those that control the flow of work. The schedule shall include review and correction periods associated with each item.
This should be a forward planning as well as a project-monitoring tool. The schedule reflects calendar days and not dates for each activity. If the schedule is changed, the Contractor shall submit a revised schedule reflecting the change within seven calendar days.
d. Contractor Review Effort. The Design Quality Control Plan shall include, but not be limited to the following contractor performed review elements:
• Design Development Review
• Criteria Review
• Presentation of Data Review
01 45 04.00 10-5
• Editing of Guide Specification Review
• Inter-Disciplinary Review
• Final Review
e. Quality Control. The professional quality, technical accuracy and the coordination of all design documents and other services to be provided by the Architect-Engineer and any subcontractors/consultants used is of major importance. It is therefore a requirement for the contractor to have a logical and functional quality control program to assure that errors and deficiencies in all submittals are minimal.
To meet this requirement, the contractor shall perform technical and inter-disciplinary reviews and shall correct all errors and deficiencies in the design documents prior to submitting them to the COE for review. The contractor’s cover letter, which transmits the design documents for review, shall include a statement of certification, co-signed by both the Design Quality Control Manager and the Design Coordination Manager, that the contractor has performed a detailed review and coordination of the submitted documents. The contractor’s performance evaluation will be based in part on how his design package reflects conformance with his DQC Plan. The contractor’s contractual obligation to provide complete, well coordinated, and error-free documents has far-reaching consequences. Therefore, the contractor is cautioned to place special emphasis on this aspect of the DQC Plan. The contractor will be held liable for such damages. The Government’s review effort in no way relieves the contractor of his contractual responsibilities.
For this reason, an effective quality control plan is critical.
f. Design Quality Control Checklists. The DQC Plan shall include the checklist and lessons learned found ER 1110-1-12 appendix G and F.
Discipline-specific checklists and lessons learned are to be used during the design and quality control of each submittal. The purpose of these lists is to provide a useful tool for the contractor to assure a quality contract package. The lists point out numerous areas which, when appropriately considered during the design, will help ensure a quality project. The lists should not be taken as covering all aspects of the project. These completed checklists shall be submitted at each design phase as part of the project documentation. If, during COE review of the final design documents, it becomes apparent that items initialed off have in fact not been completed/coordinated, the COE will notify the Prime Contractor and appropriate action taken. Possible actions include return of the final design package to the contractor for correction, withholding of contract payments and/or completion of an interim “unfavorable” contractor evaluation.
g. Design Review Check-off Matrix. In addition to the Design Quality
Control Checklist noted above, the Contractor shall prepare a Quality Control Review Matrix. This Matrix shall be developed from Section 01 10 11 Specific Engineering and Design Criteria. Each individual paragraph shall be entered into the matrix and signed off by the Construction Design Coordination Manager, Design Quality Control Manager, and the Independent Technical Reviewer, indicating that the requirements identified have been incorporated in the design documents.
