DETAIL SHEET – SPECIALIZED EXPERIENCE FOR NON-JOC

PROJECTS/CONTRACTS

Fill out one of these forms for each NON-JOC project/contract listed on Attachment 2 PROJECT NO. _____, Page 1 of ______ Submit up to 5 projects/contracts, more than 5 will not be evaluated; less than 5 may result in a lower rating; submit one of these forms for each projects/contracts submitted. This attachment is limited to a total of five (5) pages.

5. Provide detailed information of construction work for projects in at least one of the following categories, but work in more than one category may receive a higher rating. Do not just provide a listing of existing or previous contracts.

a. Building renovations that include floors, doors, walls, windows, external facades, repairs to roofs;

b. Repair/replacement of major building systems such as electrical, mechanical, HVAC;

c. Civil work, that includes concrete/asphalt roads and pavements, earthwork, and drainage structures;

d. Environmental work, such as asbestos and/or lead material abatement and disposal

Offeror: A contractor and/or company submitting a proposal for this solicitation.

Prime Contractor: A contractor that performed at least 20% of the work, and managed all other work for the project.

Key Subcontractor: A contractor that has signed a Letter of Commitment (LOC) to perform work for the duration of the contract for the Offeror, and the Offeror is allowed to take credit for the Key Subcontractor’s experience.

Joint Venture: More than one contractor with a legal binding agreement to work together.

Mentor-Protégé: More than one contractor with a legally binding Mentor-Protégé agreement that was approved by the Small Business Administration.
