About this file

This is a Past Performance Questionnaire template issued by the U.S. Army Mission and Installation Contracting Command (MICC) at Fort Eustis in support of Request for Proposal W9124G24RA001. The questionnaire is designed to solicit candid assessments of contractor performance from points of contact familiar with recent and relevant federal, state, local government, or commercial contracts comparable to the solicitation requirements. Respondents must complete and return the questionnaire within three days of receipt or by a specified deadline (MMMM DD, YYYY at 10:00 AM EDT), submitting to Contracting Officer David M. Clark (david.m.clark210.civ@army.mil) and Contract Specialist Hunter Langdon (hunter.m.langdon.civ@army.mil).

The questionnaire consists of three parts: Part I requires the offeror to identify contract details including contractor name, address, program identification, contract number, contract type, award date, completion date, nature of effort, and total contract value. Part II requires the respondent point of contact to evaluate contractor performance across five categories—effectiveness of staffing and management plan, stability of workforce, ability to provide qualified personnel for surge requirements, commitment to customer satisfaction, and general comments—using rating scales of Exceptional, Very Good, Satisfactory, Marginal, or Unacceptable, with required rationale for each response. Part III requests return of the completed questionnaire and includes sections for identifying other relevant past efforts and respondent contact information. The document is a template and does not contain specific products, services, pricing, or contract award information related to the Fort Rucker Gunnery Range Maintenance solicitation.

Very Good: Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

Satisfactory: Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
