5. Contract Name:

- The full name of the prior contract being identified to include any acronyms known.

6. Contract Number:

- The contract number of the basic contract under which the performance occurred should be called out in this section to verify the number.

8. Subcontract Number (if applicable):

- For past performance involving subcontracted activities, the identification of the subcontract number should be provided, understanding that nomenclatures vary across Government agencies.

9. If a subcontractor, provide the name of prime contractor and point of contact within the prime contractor (name, current address, email, telephone number):

The cost/price data provided should refer ONLY to the specific scope cited under #5 through #13 above. If there is no change between the awarded price/cost and the final, then the Offeror shall state “No change, reference #14 above.” If there is a change between awarded and final figures, Offerors are recommended to identify the reason for the difference in subsequent parts of this Questionnaire. Training centered on shared Tactics, Techniques and Procedures to overcome the heavily wooded desert terrain of North Africa and support the stilt-walking troop operations in support of joint C4ISR tasks executed by The U.S. Nonexistent Joint Combatant Command (USNONCOM).

19. Was this training tied to a delivery of equipment/materials/supplies (Yes or No)? If Yes, provide the dollar value of the training itself, minus equipment, materials and supplies:
