SOURCE SELECTION SENSITIVE INFORMATION – PROTECT IAW FAR 2.101 & 3.104

PAST PERFORMANCE SELF-ASSESSMENT (PPSA)

This PPSA shall include three detailed and relevant performance examples from the Offeror and one detailed and relevant example from each major subcontractor conducted within the last three years. The Offeror shall use one PPSA for each performance example identified. Section L.5 of the Solicitation should be read in its entirety before completing these PPSAs.

Instructions and samples are provided to assist the Offeror in completing this PPSA and to understand the information requested. The instructions are for formatting information only and are not intended to represent any factual information related to the ETSC. Any relation to actual entities, locations, organizations, contracts, nations, or places is merely coincidental.

In the event that any changes in corporate identity have occurred for the submitting firm (Prime or subcontractor) from the date of past performance to the date of completion of this PPSA, then the applicable questions shall be answered with both the current (first) and prior (labeled as “previously” and listed second) information (e.g. name of the company, CAGE Code, DUNS Number, address, point of contact). If the Offeror is proposing on behalf of a division or subsidiary of a larger entity, then the name of that division or subsidiary shall be provided. Claiming experience and credit for performance examples conducted by another division or subsidiary of the company, other than the one under which the Offeror performed, will be considered a misrepresentation of facts which may result in the PCO’s disqualification of the Offeror’s proposal for non-compliance.

In the event of a conflict in information requirements, the guidance in Section L.5 of the ETSC Solicitation takes precedence.

This PPSA is completed by the Offeror and provided as part of the ETSC Past Performance Volume.

23. Government Contract Administration Contact Information:

- Provide Government contract administration activity and the Administrative Contracting Officer’s (ACO) name, title, agency, email address, and telephone numbers:

Outstanding Good Acceptable Marginal Unacceptable

Outstanding: Performance meets contractual requirements and exceeds many requirements that benefit the end user. Work was accomplished with few, if any, minor problems for which corrective actions taken by the contractor were highly effective.

Good: Performance meets contractual requirements and exceeds some requirements that benefit the end user. Work was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

Acceptable: Performance meets contractual requirements. Work was accomplished with some minor problems for which corrective actions taken by the contractor were acceptable.

Marginal: Performance does not meet some contractual requirements. Serious problems with contractor performance were experienced for which the contractor has either not yet identified corrective actions or the corrective actions taken appear only marginally effective.

Unacceptable: Performance does not meet most contractual requirements. Serious problems with contractor performance were experienced for which the corrective actions were ineffective.

24-1. Self-Rating of Compliance of Services and Related Deliverables to Customer Requirements and Standards of Good Workmanship.

24-5. Self-rating of Commitment to Customer Satisfaction and Priorities.

- Identify how the firm addressed customer priorities and customer satisfaction. What efforts were undertaken, what procedures were adjusted, what new processes were created, or any other explanation of efforts to address customer satisfaction and priorities should be explored here. (Check appropriate block.)

24-6. Self-rating of How Did You Handle any Issues Related to Cost/Schedule or Performance?

- Cost, schedule, and performance are often seen as trade-offs that must be balanced to best suit an individual situation. This question seeks to learn how these balances were managed during execution of the prior work.
