About this file
This document is a standard Internal Revenue Service (IRS) Form W-9 (Revised March 2024) for Request for Taxpayer Identification Number and Certification, which is a standard tax form used by businesses and entities to provide their taxpayer identification information to requesters for information reporting purposes. The form provides comprehensive details about an entity's tax classification, including options for individual/sole proprietor, C corporation, S corporation, partnership, trust/estate, and LLC, with specific instructions for completing each section. The form requires the taxpayer to provide their name, business name (if different), federal tax classification, address, and taxpayer identification number (either Social Security Number or Employer Identification Number).
The form includes important certification statements where the taxpayer affirms their taxpayer identification number's accuracy, confirms their U.S. citizenship or person status, and attests to their backup withholding and FATCA reporting status. This version (March 2024) includes updated instructions, particularly around line 3a modifications for how disregarded entities complete the form and a new line 3b for flow-through entities to indicate direct or indirect foreign partners, owners, or beneficiaries. The form is designed to help requesters file accurate information returns with the IRS and ensure proper tax reporting and compliance.
