About this file

This is a W-9 form (Request for Taxpayer Identification Number and Certification) related to the Brick Paver Restoration project in the Siesta Key Village Public Improvement District within Sarasota County, Florida. The W-9 form is a standard IRS document used to collect taxpayer identification information from contractors and service providers before payment is issued. The form requests the contractor's name, business name, address, taxpayer identification number (either Social Security Number or Employer Identification Number), and certification of the contractor's status. This document is a prerequisite requirement for contractors bidding on or selected to perform the brick paver restoration work along Ocean Boulevard, which involves removing and resetting existing decorative pavers across approximately 800 square feet at five specified locations.

The W-9 form does not contain pricing information, set-asides, funding sources, or contract terms. Instead, it serves as a tax compliance document that contractors must complete and submit to Sarasota County to establish their legal identity and tax status. Completion and submission of the W-9 is mandatory for any contractor receiving payment from a government entity, regardless of contract value. The form certifies that the provided taxpayer identification number is correct and that the contractor is not subject to backup withholding under Internal Revenue Code regulations.
