NEXT GENERATION COMBAT VEHICLE OPTIONALLY MANNED FIGHTING VEHICLE (NGCV-OMFV) SOLICITATION W56HZV-18-R-0174
1. This information, when filled in, shall be treated as Source Selection Information in accordance with FAR 2.101 and 3.104-3 and shall not be disclosed to anyone outside the Government Source Selection Team.
Upon completion of this questionnaire, please forward by email to Aviation and Missile Research Development and Engineering Center (AMRDEC) Safe Access File Exchange (SAFE), which can be accessed at the following link: https://safe.amrdec.army.mil/SAFE. When submitting to AMRDEC, individuals need to specifically authorize access to the following:

2. Under the NGCV-OMFV Request for Proposal, offerors will propose a new combat vehicle that will be designed for and used by military forces to maneuver Soldiers in the future operating environment (FOE) to a position of advantage, in order to engage in close combat and deliver decisive lethality during the execution of combined arms maneuver. The NGCV-OMFV must exceed current capabilities while overmatching similar threat class systems. It must be optimized for dense urban areas while also defeating pacing threats on rural (open, semi-restricted and restricted) terrain and be characterized by the ability to spiral in advanced technologies as they mature.
3. Offerors interested in proposing on this solicitation have been requested to send the attached Past Performance Questionnaire to point(s) of contact on past and present contracts, which the offeror deems relevant to this effort. In this regard, Army Contracting Command – Warren (ACC-WRN) is conducting a past performance assessment in anticipation of possible contract award.
4. ACC-WRN is requesting your assistance in completing this questionnaire so that it may evaluate the offerors in the area of past performance. The attached questionnaire has been developed for ease of electronic completion. Please provide your comments regarding the overall assessment of the contractor’s performance on the contract in accordance with the instructions on the questionnaire. If a question on the questionnaire is not applicable, please so indicate.
5. Please complete and submit via email the questionnaire as soon as possible, but no later than the solicitation closing date of 01 October 2019 with the subject heading PAST PERFORMANCE INFORMATION FOR [Contractor name]. Thank you in advance for your assistance.

Exceptional: Performance meets contractual requirements to the end user or customer’s benefit. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Very Good: Performance meets contractual requirements and exceeds some contractual requirements to the end user or customer’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the contractor was effective.
Satisfactory: Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear, or were, satisfactory.
Marginal: Performance does not meet some contractual requirements. The contractual performance of the element or sub-elements being evaluated reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective, or were, not fully implemented.
Unsatisfactory: Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
