About this file
This document contains a Job Work Order and associated Fire Alarm and Emergency Communication System Inspection and Testing Forms (NFPA 72, Fig. 14.6.2.4) for three buildings (501, 502, and 503) at the Hawaii Air National Guard facility in Wahiawa and Kahului, Hawaii.
The work order (Order #240230-4, dated 03/14/2025) documents service call testing and assessment of fire alarm systems for HAWK-NIKING, LLC across three buildings performed on 3/3/25 and 3/4/25 for 8 hours total. Building 501 contained a non-operational Quick Start Panel CPU and failed BPS power supply, with multiple failed audio/visual devices including SIGA-278 modules, 270-SPO devices, and G1RF-HDVM devices. Building 502 had both a non-operational Quick Start Panel CPU and a Monaco Panel lacking power, with extensive device failures including 10 pull stations, 14 heat detectors, 1 horn/strobe, and 4 bells; the new extension warehouse had no fire alarm devices. Building 503 contained a non-operational Monaco Panel with defective power supply and zone board, plus failed pull stations, heat detectors, and bells. The three formal NFPA 72 inspection and testing forms document comprehensive system failures across all control units, power supplies, notification appliances, and initiating devices with all components marked as FAIL in visual and functional testing. The follow-up action required is to provide a proposal to replace all defective and non-operational materials, with all three buildings determined to require full fire alarm system upgrades. Phoenix Pacific, Inc. (contact 808-682-1000) performed the inspections under NICET Level II certification.
