About this file

These are fire alarm system service work orders and inspection/testing documentation for Hawk-Niking LLC facilities located at Kalaeloa, Hawaii (Buildings 1922 and 1923 at Saratoga Ave., Pearl Harbor). The documents comprise two service work orders dated November 6 and November 14, 2024, performed by Phoenix Pacific Inc., and an NFPA 72 Fire Alarm and Emergency Communication System Inspection and Testing Form dated November 14, 2024.

The first work order (240230-1, November 6, 2024, 4 hours) documents troubleshooting of a Siemens CP-35 control panel serving both buildings. Key findings include: a permanently stuck alarm on Zone 1 (repair bays, office, and sprinkler) requiring panel replacement; batteries failing load voltage tests (12V 7Ah) and requiring replacement; trouble on Zone 3 (training facility) due to open circuits and wiring issues; King Fisher radio transmission boxes missing electronics with uncertainty about box numbers (77xxx or 99xxx); and a disconnected tamper switch. The second work order (240230-2, November 14, 2024, 4 hours) continued troubleshooting in the training facility, removing four malfunctioning detectors and addressing voltage issues. Follow-up actions specified replacement of the main panel with a conventional system supporting relay output for King Fisher transmission; installation of King Fisher KF1 pushbutton and interface board; replacement of all 16 smoke detectors, 4 stick heat detectors, and 1 heat detector due to age-related line voltage degradation; and continued field wiring troubleshooting. The NFPA 72 inspection form documents system specifications (Siemens CP-35, 4 zones, 1 NAC, 120V primary power, 12V 7Ah batteries rated 24 hours standby/5 minutes alarm), test results showing battery failure, and supervisory station monitoring requirements for the Regional Dispatch Center at JBPH. All work was performed by Alston Chamizo with NICET Level II Fire Alarm Systems certification, with $0.00 billing for both service orders.

DESCRIPTION:

CALL TYPE:

WORK PERFORMED: 11/06/2024

Alston C.

4 Hours

ACTIONS TAKEN:

-Panel services both buildings.

Model Siemens CP-35 4 Zones

1 NAC

Alarm active on zone 1 (repair bays, office, and sprinkler) Trouble active on zone 3 (training facility) Battery trouble active Alarm silence and trouble silence were active on arrival

-Started checking active alarm with waterflow and verified wiring and microswitch are good. Found tamper disconnected and will need to reconnect in series with the waterflow to be monitored.

-Checked the other two pull stations on the zone and both were normal. No short found on the zone 1 to activate the alarm. Wiring was verified to be normal and replaced wiring with jumpers to rule out the field wiring. End of line capacitor was changed with zone 2s to verify the capacitor was not the problem. Alarm will not clear despite this and does not clear despite a cold restart.

*Alarm is being generated by the panel and permanently stuck and needs to be replaced.

-Need to replace 12V 7Ah batteries tested at a low voltage and does not keep power on when 120V is removed. This battery Ah may change on replacement of the panel.

-Troubleshot trouble in training facility.

-Found an open between the outgoing and return.

-Verified wire voltage on exiting the building and made a wiring correction on entering the training facility.

-Traced wiring throughout the facility checking all devices and junction boxes.

-Found a smoke detector above the ceiling in an office that will need to be brought down below the ceiling.

-Was able to verify voltage up to the area around the kitchen where one issue is suspected due to a bad voltage. Another known issue is no voltage being recieved on the return side from the panel

to this area.

-Checked on King Fisher boxes and found the electronics were removed with boxes and wiring remaining. Will need to replace the interface, KF1, and antenna with cable.

*Need to ensure frequencies will match the recieved box number through RDC. Current box number is 77xxx or 99xxx, unsure due to both numbers being written in multiple areas.

-Because panel is permanently in alarm, removed the notification devices to prevent constant reminder noise from panel. This has no increased risk of safety since panel was already in alarm and silenced and any new alarm would make no notifications to the buildings or to RDC due to the missing radio hardware. The buildings have had no fire alarm support since this zone 1 trouble appeared and will not have any until panel is replaced. RDC will not respond to any alarms until King Fisher is replaced due to missing hardware.

FOLLOW UP ACTIONS:

-Replace panel to clear zone 1 alarm. Will need a conventional panel with relay support to send signals to King Fisher radio.

-Retrieve box number from RDC to order correct frequencies for replacements of panel hardware and antenna with cable. (Interface and push button) -Continue troubleshooting field wiring in the training facility and find wiring issues and bad devices needing to be replaced.

-Terminate tamper to zone 1 wiring. May be transferred to a another zone when panel is replaced.

New wiring to the panel will be needed.

-Replace existing batteries with a possible change in Ah value after panel replacement.

WORK PERFORMED: 11/14/2024

Alston C.

4 Hours

ACTIONS TAKEN:

-Continued troubleshooting smoke detector line.

-Found multiple smoke detectors dropping the voltage on the line.

-These devices were removed and bypassed on the base. Detectors were left on top of the panel.

Three smokes and one heat were removed.

-Also addressed wiring issues while troubleshooting.

-Tested all pull stations, notification devices, and waterflow including the disconnected tamper.

Zone 1 that was stuck in alarm was moved to zone 2 temporarily to test the line.

FOLLOW UP ACTIONS:

-Replace main panel with a conventional system with onboard relays for King Fisher transmission.

-Install King Fisher KF1 (pushbutton) and interface board with batteries for both.

-Recommend replacing all detectors including bases in the training area due to their old age and causing issues on the line. Counted 16 smoke detectors, 4 stick heat detectors, and 1 heat detector. All are 2-wire conventional.
