About this file

This is a job work order and fire alarm system inspection/testing report for Phoenix Pacific, Inc. (PPI) documenting assessment services performed on February 18-19, 2025, at six buildings (701-705, 771) located at Keaukaha Military Reservation in Hilo, Hawaii for client Hawk-Niking, LLC.

The assessment identified multiple critical deficiencies requiring repairs. All fire alarm control panels (FACPs) operate as standalone systems without third-party monitoring. Secondary power supply batteries installed in 2009 are physically damaged and require replacement across four buildings: Bldg 701/703/704/705 (12v 18ah), Bldg 704 ICAP Training room (12v 33ah), and Bldg 771 (12v 7ah). A CPU RAM battery (CR2032) requires replacement in Bldg 704 ICAP. An end-of-line capacitor (50uF, 50v) needs replacement for Bldg 702 pull stations. Additionally, three notification devices (strobe horns/pull stations) must be replaced due to failure or weather damage in Bldgs 704, 705, and 771. A sprinkler riser flow switch at Bldg 702 requires replacement but falls outside PPI's scope. Recommended service includes installing a 120vac surge suppressor at the 4100U FACP and further assessment of panel interconnection capabilities. Buildings 751, 752, and 753 have no fire alarm devices. PPI will generate a comprehensive repair quote including 12 hours of technician service and all required materials/parts. The work order specifies service completion, though final quote generation and customer notification remain pending.

PANEL CONDITION ON ARRIVAL:

Panel was deenergized on arrival, reasoning is uncertain but, assumed to be due to systems condition being in trouble and not being able to silence. Systems history log was accessed and reviewed. PPI noticed several events occurred related to power outages or over currents. These are likely generated from actual events and the system may had "LATCHED" in reboot when in attempted to restart. This results in the system being in a constant cycle which would keep the trouble PSEUDO sounders activated.
