About this file

This is a Vendor Affidavit document issued by Hillsborough County Public Schools (HCPS) in Florida regarding compliance with anti-human trafficking requirements under Florida Statutes Section 787.06(13). The affidavit is required for all nongovernmental entities executing, renewing, or extending contracts with HCPS. The document relates to an Electrical Services solicitation through which HCPS seeks qualified contractors to provide commercial electrical services, including new installation, maintenance, repairs, electrical upgrades, modernizations, circuit breaker installations, panel upgrades, troubleshooting, and diagnostics on high voltage systems. Contractors must demonstrate experience with switchgear, high voltage systems, coordination with TECO, and the ability to service a school district of HCPS's size, as electrical services are identified as critical to campus security.

The affidavit requires an authorized company officer or representative to certify under penalty of perjury that the contractor does not engage in coercion for labor or services, including use of physical force, unlawful restraint or confinement, debt-based labor arrangements, document confiscation, financial harm threats, fraud or deception, or provision of controlled substances for exploitation. The vendor must sign, print their name and title, and date the affidavit before submission. This compliance documentation is a prerequisite for contract award and must be completed in conjunction with the submission of proposals for electrical services.
