Report of asbestos sampling of suspected asbestos-containing materials in the third floor IRM project area at the Louis Stokes VA Medical Center located at 10701 East Boulevard, Cleveland, Ohio 44106 (22881)
Dear Mr. Huffman;
On January 10, 2019, Stephen J. Sebesta and Associates, Inc. collected samples of suspected asbestos-containing materials in the third floor IRM rooms at the Louis Stokes VA Medical Center located at 10701 East Boulevard, Cleveland, Ohio 44106. This sampling was conducted to identify any asbestos-containing materials which may be impacted by the proposed “Increase IRM Security Project”. This letter provides a summary of the samples collected and laboratory results.
The proposed renovation includes relocating walls, installing new doors, and removing and replacing flooring, ceilings, doors, and other work in the third floor IRM Rooms 3A-133, 3A-135, 3A- 135A, 3A-135B, and 3A-135C as shown on the Perspectus Architecture Drawings AS.1, AS.2, AS.3, and AS.4. The IRM area is located on the east end of the third floor of the main building. The rooms comprise an approximately 2,800 sq ft area. The main building was constructed around 1961.
METHODOLOGY
The sampling was conducted in accordance with the USEPA guidelines recommended for public and private elementary and high schools to meet the requirements of the Asbestos Hazard Emergency Response Act (AHERA) of 1986 and the National Emission Standards for Hazardous Air Pollutants (NESHAP) requirements for the renovation and demolition of buildings containing asbestos-containing materials (ACM).
Based on the age of construction, physical appearance, location, systems where located and other factors, flooring materials were identified as homogeneous asbestos-containing materials.
Samples of the suspect homogeneous Materials were sampled and analyzed for asbestos by a NVLAP accredited laboratory in accordance with AHERA requirements.
Samples were collected in 6 mil zip-lock polyethylene bags and the sample number as well as other related information, was marked on the sample bag label. Where feasible, sample numbers were also marked adjacent to the field sample location. All sample bags were placed in a second 6 mil polyethylene bag and submitted to the laboratory for analysis along with a chain-of-custody form.
Attachment No. 1 contains an Asbestos Bulk Sample Log showing a summary of the analysis of bulk samples obtained during the survey and copies of the Environmental Hazards Services (EHS) bulk sample laboratory reports for the bulk asbestos samples collected on this inspection. Attachment 2 contains excerpts from the “Asbestos Survey Report, Main Building, Louis Stokes VA Medical Center” dated 10-29-15. Attachment No. 3 contains photographs of the sample locations. Attachment No. 4 contains the Ohio Asbestos Certification and Training Information for the asbestos inspector involved in the survey and preparation of the report.
A total of 23 additional bulk samples (30 layers) were collected by a certified inspector and analyzed by a NVLAP accredited laboratory. The results of the laboratory analysis of these samples are included in Attachment No. 1. Homogeneous materials sampled in the survey included ceiling tiles, drywall, wall plaster, tar over fiberglass pipe insulation, floor tiles and mastic, cove base molding and mastic, adhesive below raised flooring stanchions, duct mastic, and firestopping.
A total of 615 bulk samples (718 layers) were collected by a certified inspector and analyzed by a NVLAP accredited laboratory on a previous inspection throughout the main building. Excerpts from the “Asbestos Survey Report, Main Building, Louis Stokes VA Medical Center” dated 10-29-15 are included in Attachment No.2.

IRM AREA)
Much of the existing ductwork is insulted with fiberglass. It is possible that some of the existing older ductwork has the older red mastic below the insulation. Sample Nos. 19341-543 19341-781b, 19341-832a, 19341-727, and 19341-771 of the red duct mastic found on seams of older original ductwork throughout the main building collected on a previous inspection were reported to contain 1.25-5% chrysotile asbestos by PLM. Sample No. 19341-1285 of the red mastic collected on a previous inspection were reported as NAD (No Asbestos Detected) by PLM.
The older red duct mastic, if present, on the older ductwork below the fiberglass is a Category 1 non friable material. If the older red duct mastic is found, it should be assumed to contain asbestos.
NON ASBESTOS-CONTAINING RED FIRESTOPPING (THROUGHOUT 3RD FLOOR IRM AREA)
Sample No. 22881-21 of the red firestopping in the Third floor IRM rooms were reported as No Asbestos Detected (NAD).
