About this file
This is an Insurance Checklist document issued by the City of West Sacramento detailing Certificate of Insurance (COI) requirements for standard city contracts, specifically for the US 50/BUS 80 Community Connectivity Project. The document establishes mandatory insurance coverage requirements that all contractors and vendors must maintain and provide proof of before contract execution. Key insurance requirements include Commercial General Liability coverage of $2,000,000 per occurrence/$4,000,000 aggregate, Commercial Automobile Liability of $2,000,000 per accident, Workers' Compensation per statutory requirements, and Professional Liability (Errors & Omissions) of $2,000,000 per claim if applicable. The City of West Sacramento, its officers, officials, employees, agents, and volunteers must be named as additional insureds on all policies except Workers' Compensation and Professional Liability. All certificates must provide thirty (30) days' written notice to the Certificate Holder prior to cancellation or material changes.
All required policies must include specific endorsements to comply with City standards. General Liability and Automobile Liability policies require Additional Insured Endorsements equivalent to CG 2010 0714, Primary and Noncontributory Endorsements stating the insurance is primary and noncontributory, and Separation of Insureds clauses applying coverage separately to each insured. Workers' Compensation policies must include a Waiver of Subrogation Endorsement in favor of the City and its officers, agents, and employees, or a Workers' Compensation Waiver if the contractor is self-employed with no employees. Contractors must forward this checklist to their insurance agents or brokers and submit copies of all Additional Insured Endorsements with their Certificates of Insurance to the City at 1110 West Capitol Avenue, West Sacramento, CA 95691, with incomplete certificates subject to rejection.
