About this file

This document appears to be a government handbook section on Contract Discrepancy Reports (DA Form 5479). It outlines the government's process for documenting and addressing defective contractor performance under a contract, including providing the contractor an opportunity to repair or replace nonconforming services. The handbook references the use of various surveillance documentation forms to monitor contractor performance. It cites the FAR requirements for handling nonconforming supplies or services, including notifying the contractor and requesting corrective action at no cost to the government.

The related federal contract opportunity is for the development of test items for the Defense Language Proficiency Test (DLPT5), which is used to assess the foreign language capabilities of military and government linguists. The Department of the Army Materiel Command Mission and Installation Contracting Command Fort Eustis has a need for 5,000-7,500 DLPT5 test items annually, covering approximately 350 languages and dialects across multiple proficiency levels. The contract will require the contractor to create reading and listening test items that conform to DLPT item development specifications.
