About this file
This is a Sales and Use Tax Construction Contract Exemption Certificate form issued by the State of Ohio for use by the City of Toledo in connection with the TPD Alarms Building Renovation project. The certificate enables contractors and subcontractors to purchase tangible personal property and services exempt from Ohio sales and use tax when those materials and services are incorporated into eligible real property construction projects. The form requires signatures from the owner/contractee, government officials, prime contractors, and subcontractors, with original documentation retained by the prime contractor and copies maintained by all parties involved in the procurement. Eligible properties for exemption include buildings used exclusively for charitable purposes by nonprofit organizations, horticultural or livestock structures, sports facilities, hospital facilities, computer data centers, megaprojects, real property owned by the United States government or State of Ohio, buildings under construction contracts with tax-exempt organizations, houses of public worship, and convention centers that qualify for property tax exemption.
The certificate identifies the City of Toledo as the political subdivision, with the Commissioner of Purchases and Supplies located at One Government Center, Suite 2000, Toledo, Ohio 43604, and is effective January 1, 2026. Bidders submitting proposals for this project must submit the completed tax exemption certificate along with other required documentation including construction project paperwork, line items, bid guaranty, signed contract, and proof of tax exemption status through the City of Toledo's PlanetBids portal. The bid guaranty must be in the form of a cashier's check or bid bond made payable to the City of Toledo and received by the Division of Purchases and Supplies by the bid due date, or the submission will be deemed non-responsive.
