About this file

This document comprises comprehensive screening instructions for the New Jersey Division of Revenue and Enterprise Services (DORES) related to front-end mail receipt and pre-processing services. The instructions establish standardized procedures for processing various New Jersey tax and fee forms, including income tax returns (NJ-1040, NJ-1040-NR, NJ-1040-H, NJ-1040X), fiduciary returns (NJ-1041, NJ-1041-SB), partnership returns (NJ-1065), corporation business tax returns (CBT-100, CBT-100S), property tax reimbursement applications (PTR-1, PTR-2), and numerous specialized forms related to sales tax, employment taxes, motor vehicle registration, pharmacy assistance programs, and other state revenue collection mechanisms. The instructions detail verification protocols for Social Security numbers, names, addresses, Federal Identification Numbers, income reporting, tax withholdings, proof of payment submissions, and age or disability benefit documentation. Processing requirements include date stamping all documents with postmark or received dates, applying appropriate division use codes, removing staples and clips, preparing documents in specified orders, and routing forms to appropriate processing centers including scanning, control, look-up, or out-sort destinations based on completeness and form type.

The screening instructions establish validation procedures for matching document amounts with check amounts, adjusting figures within specified tolerance ranges without exceeding acceptable variance thresholds, and handling missing pages through insertion of developer preparation pages or outsourting to appropriate processing divisions. All documents must be initialed by screeners in designated locations, typically the lower right-hand corner, and returns organized by tax year (2014, 2013, 2012, and 2011 or prior) with applicable separator sheets and patch pages. Special handling protocols address amended returns, extensions, multiple submissions with calculator tapes, checks written in red ink, and forms with missing critical information, with specific instructions for voucher preparation across multiple revenue categories and mailbox locations. The instructions emphasize strict adherence to document preparation standards without making unauthorized alterations to taxpayer entries except where expressly permitted, maintaining consistent coding practices across all processing functions.

They should be initialed, prepped and boxed for scanning!!

Do not code until screening is completed for the primary form and attachments!!!

Step Action

1 ALL 2014 NJ-1040 RETURNS (with or without NJ-1040H) & PRIOR YEARS

A. Check for a label at top of the return. If label is attached go to Step B.

When there is NO LABEL the following MUST be entered:

1. Social Security number (9 digits) MUST be entered.

2. Name, Street, City and State MUST be entered. If all of the above items are not filled in, copy information from W-2 form.

B. If a check is attached, prepare a NJ-1040-VM (see instruction for “Vouchers”).

C. All W-2s MUST be the same year as the 1040.

1. W-2s TP must match the TPs on the primary return.

D. Line 14 Compare State wages (line 16) and Federal wages (line 1) on each W-2. If the figure reported on Line 14 is greater than total W-2 amount, accept as a “Good Return”. NEVER DECREASE WAGES. If different, add up state wages from W-2s:

1. If the total wages calculated are within $500 of the amount reported on Line 14, process as received.

2. If the total wages calculated is more than $500 greater than the amount reported on Line 14, line out amount on Line 14, place calculated figures to the left of the line and place “J” code in box 5, located at the bottom of page 1 in the “Division Use” area.

3. NEVER DECREASE LINE 14. Increase wages if the calculated value is more than $500.

E. If there is an amount on Line 46, confirm that form NJ-2210 has been enclosed with the NJ-1040. If the form NJ-2210 has been enclosed, screen and code appropriately (see screening instruction for NJ-2210). If not enclosed zero out line 46.

F. Line 48

Verify that all claimed withholdings (found on the W-2’s, box 17) are New Jersey withholdings and NOT New Jersey Unemployment withheld (FLI) or withholdings for another state. Add up New Jersey withholdings from all W-2s, W-2Ps and/.or 1099Rs. If the withholding figure does not agree with the amount on Line 48:

1. Re-check addition of New Jersey withholdings;

2. Search return and attachments for any possible missing W-2s, W-

2Ps or 1099Rs;

3. If a discrepancy still exists between Line 48 withholdings and the total state tax withheld on W-2s, W-2Ps or 1099Rs, compare total wages reported on Line 14 to that of the total calculated on all W- 2s, W-2Ps and 1099Rs:

a. Changes are not required for $10 tolerance.

b. If calculated wages DO NOT AGREE with Line 14 amount, assume that a W-2, W-2Ps or 1099Rs is missing. If the calculated wages from the W-2s are greater than Line 14 amount and total withholdings from the W-2s are less than Line 48 amount, reduce Line 48 to match calculated withholding amount from W-2s, then place an “X” code in box 5, located at the bottom of page 1 in the “Division Use” area.

c. NEVER INCREASE LINE 48. Decrease withholdings if the calculated withholding amount is less than the amount provided by the TP.

4. Handwritten W-2 forms are not accepted if received with a 1040 web return (noted with a “CF” in the barcode). If receive, assume that a W-2 is missing and follow the instructions detailed in 3b above.

5. If the NAME OF STATE block of the W2, W-2P or 1099R is blank and either the employer or the employee address is NOT a New Jersey address, taxes cannot be verified as withheld for New Jersey. Cross out Line 48 and place correct amount withheld to the left of the boxes, and then place an “X” code on line 5, located at the bottom of page 1 in the “Division Use” area.
