About this file
This document is the Appendix A Master Quality Assurance Project Plan (QAPP) Minimum Requirements for the New Jersey Department of Environmental Protection (NJDEP) Non-Emergency Remedial Action Services (NERAS) contract. The QAPP outlines the requirements for the contractor's sampling plan, sample handling and custody, field instrument and equipment testing and calibration, data management, and analytical methods. It specifies that the contractor must solicit competitive bids for analytical services and submit the Master QAPP within 30 calendar days of the analytical services change order approval. The QAPP must detail sample collection and preservation procedures, analytical methods, field quality control, and data documentation and reporting requirements. It also requires the contractor to provide a table with information on laboratory certifications, sample containers, preservation, and holding times for any analyses requiring NJDEP certification.
The NERAS contract is for non-emergency remedial action services in Mercer County and the City of Trenton, New Jersey. The contract term is not specified. Pricing terms, set-asides, incumbents, funding sources, and other salient information are not provided in the given document.

T2658 - NJDEP Non-Emergency Remedial Action Services
Appendix A
Master Quality Assurance Project Plan (QAPP) Minimum requirementS A. The Contractor’s Master QAPP is the prime document governing sample collection, sample analysis, and data generation for environmental and waste samples. It encompasses the activities of certified laboratories providing services under this contract and Contractor staff measuring “analyze immediately” parameters in the field. The Contractor and the approved certified laboratories that will be used during the contract agree that all sampling, analysis, and data generation will be based on the approved version of the Master QAPP. If a laboratory is added after contract award, the new laboratory must agree to and comply with the approved Master QAPP.
B. The Contractor is required to solicit competitive bids for analytical services post award, and the Master QAPP shall be submitted within thirty (30) calendar days following approval of the analytical services change order resulting from these bids. The Contractor shall submit two hard copy printed copies and one electronic copy of the Master QAPP to the State Contract Manager for review and approval. The electronic copy shall be submitted as an unprotected Microsoft Word document and shall contain a separate file for each section identified below:
1. Title Page
Include company name and address, contract name and number, and revision number and date.
2. Table of Contents
Identify each section and subsection and list the appendices.
3. Signatory Page and Revision List
Include signature and date lines for the Contractor Contract Manager preparing the document, the Contractor Contract Managers reviewing the document (such as the program manager and the quality control officer. Note that the quality control officer must be independent of the unit generating the analytical data), and the State Contract Manager approving the document. List the revisions by number and date at the bottom of the page.
4. Introduction and Project Organization Chart
Explain the purpose of the QAPP. Include a project organization chart for Contractor personnel that may work on the contract and identify the personnel by name and title.
5. Project Description
Identify the scope of services that will be provided under this contract.
6. Special Training Requirements and Certifications
This section discusses Contractor personnel training and certifications. Identify health and safety training, task hazard assessment training, professional development or specialized training, and licenses or certifications possessed by staff.
7. Sampling Plan
The sampling plan shall address post excavation soil sampling, waste disposal sampling, initial waste characterization (HazCat) and bulking tests, and asbestos sampling. The plan shall detail sample collection and preservation procedures, sample transport procedures, analytical methods, field equipment standard operating procedures manuals (or references thereto) that include proper usage and calibration procedures, equipment decontamination procedures, field quality assurance/quality control procedures and data documentation and reporting requirements. Samples shall be collected in accordance with the NJDEP Field Sampling Procedures Manual or a sampling method specified by the analytical method. Actual sample collection procedures shall be described in the Master QAPP and shall not be included by reference only. The required analytical methods are identified in Section 4.5.10, and variation from the required methodology is not permitted. The Master QAPP must identify the methods by number, date, and regulatory citation (as appropriate). If a method allows the user to select from various options, the method citation should state exactly what options are being selected. Laboratory SOPs shall be available for review upon request.
8. Sample Handling and Custody Requirements
Describe field documentation, sample labeling, sample chain of custody, field and laboratory QA/QC procedures, and specific sample quality control procedures. Sample handling provisions must take into account the nature of the samples, maximum allowable sample holding times before extractions or analysis, and the available shipping options and schedules. Include examples of sample labels, chain of custody forms, and sample custody logs.
9. Field Instrument and Equipment Testing, Maintenance and Inspection Requirements
Describe frequency and methods for testing, maintenance, inspection, and calibration of field instruments and equipment. Specifically identify the instruments and equipment requiring periodic calibration and maintenance. Describe how deficiencies will be corrected. Identify documentation procedures.
10. Data Management
Discuss field data recording, data validation, data transformation, reduction, tracking, storage and retrieval. Describe or reference standard recordkeeping procedures, document control systems, and the approach used for data storage and retrieval on electronic media. Discuss the control mechanism for detecting and correcting errors and for preventing loss of data during data reduction, data reporting, and data entry to forms, reports, and databases. Provide examples of any forms or checklist to be used. Identify all data handling equipment and procedures to process, compile and analyze the data, including any required computer hardware and software. Address any specific performance requirements and describe the procedures that will be followed to demonstrate acceptability of the hardware / software configuration.
11. Analytical Methods Corrective Action Requirements
Identify all data handling equipment and procedures to process, compile and analyze the data, including any required computer hardware and software and procedures used for review (verification and usability procedures) including data assessment versus stated data quality objectives of the laboratory data. Address any specific performance requirements and describe the procedures that will be followed to demonstrate acceptability of the hardware / software configuration.
12. Documentation and Records
Describe chain of custody forms, New Jersey regulatory reporting formats, and submission of laboratory deliverables, including electronic deliverables. Itemize the information and records that must be included in the data report package and specify the desired reporting format for hard copy and electronic forms, when used.
C. For any analyses that require NJDEP Environmental Laboratory certification, the Contractor must submit a table in the Master QAPP that includes the information contained in this paragraph. It is recommended that the Contractor obtain this information directly from its certified laboratory or laboratories. If the Contractor is certified to self-perform “analyze immediately” parameters, the Contractor must prepare a table for its portion of the work. Each table must contain the following information: (a) Name of Contractor and laboratory; (b) Preparation date; (c) Parameter or method required; (d) Sample matrix; (e) Container type (include number of containers per sample); (f) Volume required per container; (g) Preservation requirements; and (h) Holding time requirements (includes extraction and analytical holding times).
D. The Master QAPP may be subsequently modified through the submission of a site-specific QAPP. Should this occur, the Contractor is required to submit a revised Master QAPP to the State Contract Manager for review and approval.
