About this file
This is a Supplemental Bidder Attestation form issued by the Rhode Island Division of Purchases on behalf of the Rhode Island Department of Emergency Management for the Mobile Emergency Operations Center (MEOC), Type 1 procurement. The form requires bidders to certify that their organization and any parent corporations, subsidiaries, affiliates, and subcontractors do not have conflicts of interest as defined under Rhode Island General Laws Chapter 14 of Title 36 with any official, officer, or agency involved in the request for proposal. Bidders must also attest that they did not materially participate in or receive consultation regarding the RFP requirements, technical aspects, or formation. A pre-proposal conference is scheduled for Thursday, November 5, 2026, at 10:00 a.m. via Microsoft Teams, with registration required by email to Judith.Ruggiero@purchasing.ri.gov.
The attestation form clarifies that being a current or past vendor of the State does not automatically create a disqualifying conflict of interest, and formal responses to Requests for Information (RFI) do not constitute participation in subsequent RFPs. The form requires authorized signatories to complete all blank fields including vendor name, date, solicitation number and title, and provide printed name and signature. The document establishes the formal certification requirements that bidders must meet as a condition of proposal submission for the Mobile Emergency Operations Center procurement.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the
State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
