About this file
This is a Supplemental Bidder Attestation form issued by the Rhode Island Division of Purchases for use in state procurement processes. The form is required under R.I. Gen. Laws 37-2-13.1(b) and applies to the DEM Reservation and Registration Management System solicitation. Bidders and their parent corporations, subsidiaries, affiliates, and subcontractors must certify that they have no conflicts of interest with any official, officer, or agency overseeing the request for proposal and that they did not materially participate in or consult on the requirements, technical aspects, or formation of the solicitation.
The attestation form requires an authorized signatory to confirm under penalty that the provided information is accurate and that the signatory is authorized to make such certification on behalf of the bidding entity. The form clarifies that being a current or past vendor to the State does not automatically constitute a disqualifying conflict of interest, and that formal responses to prior Requests for Information do not constitute participation or consultation that would disqualify a vendor from future solicitations. Bidders must complete all blank fields including entity name, authorized signatory information, date of execution, and the applicable solicitation number or title before submission.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the
State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
