About this file

This is a Supplemental Bidder Attestation form issued by the Rhode Island Division of Purchases for the Executive Office of Housing's Strategic Plan Development for Homeless Response solicitation. The attestation requires bidders to certify that their entity and all affiliated parties (parent corporations, subsidiaries, affiliates, and subcontractors) have no conflicts of interest with any state official, officer, or agency involved in the Request for Proposal, and that they have not materially participated in or been consulted regarding the RFP's requirements, technical aspects, or formation. The form further requires attestation that the solicitation does not pertain to any audit, examination, independent verification, review, or evaluation of the bidder's work, financials, or operations conducted on behalf of the State of Rhode Island or any related state entity.

The attestation must be completed and signed by an authorized signatory of the vendor or entity submitting a proposal for the EOH Strategic Plan Development solicitation. The form clarifies that being a current or past state vendor does not automatically constitute a conflict of interest requiring preclusion from participation, and that formal responses to prior Requests for Information do not constitute participation or consultation that would disqualify a vendor from future RFP opportunities. All bidders must complete the requisite fields including vendor name, date, authorized signatory name and signature, and the solicitation number or title before submission.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the

State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
