About this file
This is a Supplemental Bidder Attestation form from the State of Rhode Island Division of Purchases, required for participation in the Master Price Agreement (MPA) 421 Statewide School Food Services Program solicitation administered by the Rhode Island Department of Education (RIDE). The form serves as a certification mechanism whereby authorized company representatives must attest that their firm, including parent corporations, subsidiaries, affiliates, and subcontractors, have no conflicts of interest with any state official, officer, or agency involved in the RFP, and have not materially participated in or been consulted regarding the formation and promulgation of the solicitation. The attestation must be completed and signed by an authorized company signatory, with the firm name, solicitation number/title, and submission date clearly identified on the document.
The attestation form clarifies that prior vendor status with the State does not automatically constitute a disqualifying conflict of interest, and that formal responses to Requests for Information (RFI) do not constitute participation that would preclude future bidding on related RFPs. This supplemental form is a mandatory submission requirement for all bidders responding to MPA 421, which is seeking qualified firms to provide comprehensive statewide school food services program administration beginning July 1, 2026, with a contract term of one year plus options for four additional 12-month renewal periods. The form was revised as of November 29, 2024, and must be submitted electronically through Ocean State Procures by the proposal deadline of March 27, 2026 at 2:00 PM EDT.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the
State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
