Supplemental Bidder Attestation Summary

This is a Supplemental Bidder Attestation form issued by the Rhode Island Division of Purchases under R.I. Gen. Laws 37-2-13.1(b), applicable to the DOIT - Managed Application Services Request for Proposal (RFP27006832). The form requires authorized representatives of bidding entities to certify that their company and all associated entities—including parent corporations, subsidiaries, affiliates, and subcontractors—do not have conflicts of interest with any state officials, officers, or agencies involved in the RFP procurement process. Bidders must attest that they have neither materially participated in nor been consulted regarding the requirements, technical aspects, or formation of the RFP. The attestation also confirms that the RFP does not involve audits, examinations, or evaluations of the bidder's work, financials, or operations on behalf of the State of Rhode Island.

The form clarifies that being a current or past state vendor does not automatically create a disqualifying conflict of interest, and that formal responses to prior Requests for Information do not constitute participation or consultation that would preclude future RFP participation. Authorized signatories must complete all required fields including printed name, signature, solicitation number and title, vendor/entity name, and date. This attestation must accompany proposals submitted for the Managed Application Services contract, with services commencing January 1, 2027, for an initial three-year term with options for two additional 24-month renewal periods.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the

State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
