About this file
This is a Subcontractor Utilization Plan form prepared for the City of St. Petersburg, Florida's Anti-Gun Violence Initiative peer support and advocacy program (RFQ No. 26-169). The solicitation requires proposers to submit comprehensive peer support and advocacy services including one-on-one peer support for survivors, family and caregiver support, peer-facilitated support groups, hospital and community-based follow-up support, crisis stabilization, systems navigation assistance, community healing activities, and survivor leadership development. The program must launch within 30 days of contract execution. Qualified vendors must demonstrate a minimum of five years of applicable experience in completed projects within the past five years, preferably holding bachelor's degrees or higher in relevant fields such as public administration, social work, or community development. The submission deadline is March 19, 2026, at 3:00 p.m., with a questions deadline of March 16, 2026. All responses must be submitted electronically through the OpenGov Procurement portal as a single continuous PDF document not exceeding 50 pages excluding cover pages and dividers. The initial contract term is one year, with the City retaining options to renew for up to two additional one-year periods contingent upon satisfactory vendor performance.
The Subcontractor Utilization Plan requires proposers to voluntarily demonstrate efforts in engaging small business enterprises (SBEs), women-owned business enterprises (WBEs), veteran-owned businesses, and minority-owned business enterprises (MBEs) in the Tampa Bay Region, with no minimum participation level required. Subcontractors may be utilized for up to 80 percent of the contract work, and if used to meet SBE percentage requirements, they must be replaced with other SBE subcontractors if substituted unless the prime contractor demonstrates alternative means of meeting requirements. Business enterprise program outreach and utilization is weighted at 15 percent of the evaluation criteria. Vendors must maintain comprehensive insurance coverage including commercial general liability of $1,000,000 per occurrence and $2,000,000 aggregate, commercial automobile liability of $1,000,000 combined single limit, and workers' compensation as required by Florida law. The solicitation does not specify a total funding amount, and vendors must submit detailed quotes with unit prices and cost analyses for evaluation. The City reserves the right to reject all submissions, award on a split-order or individual basis, negotiate with the most qualified firm, and terminate the agreement for convenience upon thirty days' written notice.

Form C Subcontractor Utilization Plan Project ID:
Contractor Name:
Subcontractor Utilization Plans require proposers to show intent to use minority, women, veteran, and small businesses in the Tampa Bay Region.
Proposers must describe their efforts in engaging and reducing any barriers to participation by small/veteran/minority/women business enterprises, including outreach, education, mentorship, and process changes designed to increase small/veteran/diverse business participation.
The small/veteran/minority/women business goals set forth herein are voluntary. Proposers will not be considered non-responsive if the Subcontractor Utilization Plan has a zero small/veteran/diverse business participation amount. The City, however, encourages proposers to be proactive in engaging small/veteran/minority/women business enterprise participation. No minimum level of small/veteran/ minority/women business enterprise participation is required as a condition for receiving a Contract award.
