About this file

This is a Subcontractor Summary Worksheet for the City of St. Petersburg related to the Anti-Gun Violence Initiative – Peer Support & Advocacy program (Safe Futures St. Pete). The worksheet serves as a mandatory submission requirement with bid proposals and outlines the procedures for identifying and managing subcontractors on the project. Bidders must complete the form by entering their company name as the prime contractor, identifying the individual completing the form, and listing all intended subcontractors with their business addresses, contact information, trade classifications, and proposed subcontract amounts. The document specifies that subcontractors may not be changed or substituted after contract award and pre-construction meeting without documented justification, including failure to execute contracts, lack of appropriate licensing, bankruptcy, ineligibility, performance failures, unsatisfactory work quality, or disruption of project progress.

The worksheet establishes that up to 80 percent of the contract work may be subcontracted for shelter-related components of the project. Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substituted, unless the prime contractor demonstrates that SBE requirements are being met through alternative means. The document permits only first and second tier subcontractors unless otherwise specified in contract documents. Requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management department, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org. The worksheet includes a detailed legend of specific reason codes for deletion or addition of subcontractors and requires city approval documentation for any changes made.
