About this file

This document is a Subcontractor Summary Worksheet template issued by the City of St. Petersburg, Florida for use in responding to government procurement opportunities. The worksheet must be completed and submitted with all bids and requires prime contractors to identify all intended subcontractors, including their business addresses, contact information, trade classifications, and subcontract amounts. The City permits subcontracting up to a specified percentage of the total contract value, with restrictions on multiple-tier subcontractors except for FDOT LAP or federally funded projects. The worksheet accommodates tracking of both Small Business Enterprise (SBE) and Minority/Women Business Enterprise (MWBE) subcontractors and includes provisions for documenting changes to the subcontractor list after contract award through written requests to the Procurement & Supply Management office.

Subcontractor substitutions are prohibited after contract award and the pre-construction meeting except under seven specific circumstances, including contractor failure to execute a written agreement, lack of appropriate licensing, bankruptcy, insolvency, ineligibility, failure to perform work, substantial unsatisfactory performance, or substantial project delay. SBE subcontractors used to meet percentage requirements must be replaced with other SBE subcontractors if changes are necessary. All requests for subcontractor modifications must be submitted in writing to the Contracts Compliance Manager at the designated email address, with the City retaining approval authority over any additions or deletions documented with associated reason codes and approval dates.
