About this file

This is a Letter of Intent to Perform as a Subcontractor/Supplier form used by the City of St. Petersburg, Florida for its Lead Reduction Program procurement. The form is completed by certified business enterprises (CBEs) intending to serve as subcontractors or suppliers on the City's contract to supply lead safety equipment and materials. The procurement establishes a one-year, as-needed contract for lead-safe work practice equipment, contractor safety supplies, assembled homeowner lead safety kits, and related materials. Estimated quantities include approximately 120 pairs of contractor safety boots or safety shoes, 120 EPA Renovation, Repair and Painting (RRP) Compliance Kits, 100 assembled Homeowner Lead Safety Kits, and 25 Mobile Containment Units. The City reserves the right to increase or decrease quantities based on program needs and notes that items within kits may be subject to change. Actual quantities purchased will depend on program participation, funding availability, and operational needs.

The Letter of Intent form requires certified business enterprises to identify their applicable certifications, including Small Business Enterprise (SBE), Minority Business Enterprise (MBE), Woman Business Enterprise (WBE), or other designations. Subcontractors and suppliers must provide detailed information regarding the work to be performed, anticipated time frame, dollar amount, work start date, and certifying agency. The form requires signatures from both the certified firm's official and the prime contractor's official and includes an acceptance section for the City of St. Petersburg. This form must be completed and signed for each certified firm utilized on the project, or alternatively, Form B must be completed if no certified firms are being used or if business enterprise goal percentages have not been met.
