About this file
This is a Business Enterprise Good Faith Efforts form (Form B) issued by the City of St. Petersburg, Florida, in connection with a lead safety equipment and materials supply contract. The form is completed by prime contractors who have not met required Business Enterprise participation goals or have not utilized any Business Enterprise subcontractors for the project. The contract is structured as a one-year, as-needed agreement with anticipated purchases including approximately 120 pairs of contractor safety boots or shoes, 120 EPA Renovation, Repair and Painting (RRP) Compliance Kits, 100 assembled Homeowner Lead Safety Kits, and 25 Mobile Containment Units to support the City's Lead Reduction Program. The City reserves the right to increase or decrease quantities based on program needs, and items within kits may be subject to change.
Form B requires contractors to document their good faith efforts to achieve Business Enterprise participation through specific actions including conducting at least one monthly outreach event, documenting all solicited responses and quotes from Business Enterprises, providing a list of unavailable Business Enterprises with supporting documentation, attending Office of Supplier Development outreach events, and effectively utilizing available SBE recruitment services. The Office of Supplier Development can be contacted at 727-892-5351 or OSD@stpete.org for assistance. Contractors must provide a detailed written explanation with supporting documentation demonstrating good faith efforts if they have exhausted all options to secure Business Enterprise participation but were unsuccessful. Any material misrepresentation on the form is grounds for initiating action under federal or state laws concerning false statements.
