About this file

This is a Subcontractor Summary Worksheet instruction document from the City of St. Petersburg, Florida, applicable to the Anti-Gun Violence Initiative – Peer Support & Advocacy program (RFQ No. 26-169). The worksheet must be completed and submitted with all bids and establishes that subcontractors may perform up to 80 percent of the contract work. Prime contractors are required to list all intended subcontractors on the form, providing complete legal names, primary business addresses, contact information for project supervisors, trade classifications, and subcontract amounts. The instructions clarify that subcontractors cannot be changed or substituted after contract award and pre-construction meeting except under specific circumstances including failure to execute contracts, lack of appropriate licensing, bankruptcy or insolvency, ineligibility, poor performance, or substantial project delays. Any Small Business Enterprise (SBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substituted unless the prime contractor demonstrates that SBE requirements are being met through alternative means.

The worksheet requires documentation of all subcontractor changes through written requests to the Procurement & Supply Management division at Stephanie.Swinson@stpete.org, with approval and date notation by city personnel. Bidders who do not intend to use subcontractors must provide a written statement indicating such on the form. The document includes seven specific reasons for allowable subcontractor substitutions: failure or refusal to execute contracts at specified prices, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure or refusal to perform subcontract obligations, substantially unsatisfactory work not in accordance with plans and specifications, or substantial delays or disruption to project progress. The worksheet contains fields for tracking whether subcontractors are SBE or MWBE certified, adding or deleting subcontractors with corresponding reason codes, and city approver authorization with dates.
