About this file

This is a Subcontractor Summary Worksheet instruction document from the City of St. Petersburg, Florida, establishing guidelines for subcontractor usage on the Flowserve Limitorque L120-10 Electric Multi-turn Valve Actuator Replacement project at the North East Water Reclamation Facility. The worksheet must be completed and submitted with all bids and establishes that up to 80 percent of contract work may be subcontracted. Prime contractors must list all intended subcontractors with complete legal names, business addresses, contact information, trade classifications, and subcontract amounts. The document specifies that subcontractors cannot be changed or substituted after contract award and the pre-construction meeting, except under seven defined circumstances: subcontractor failure to execute a contract, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure to perform the subcontract, substantially unsatisfactory work performance, or substantial delays to project progress.

Requests for subcontractor substitutions must be submitted in writing to the City's Procurement & Supply Management department, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org. The document identifies whether subcontractors are Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) contractors and includes tracking columns for additions or deletions with associated reason codes and city approval signatures. If no subcontractors will be used, prime contractors must submit a written statement to that effect. SBE subcontractors used to meet percentage requirements must be replaced with other SBE subcontractors if substitution becomes necessary, unless the prime contractor demonstrates that SBE requirements are met through alternative means.
