About this file

This is a Subcontractor Summary worksheet instruction document issued by the City of St. Petersburg, Florida for use in state and local contract bidding. The document establishes procedures and requirements for prime contractors submitting bids on city projects to disclose and manage subcontractor utilization. The worksheet must be completed and submitted with all bids and requires prime contractors to identify all intended subcontractors, provide their complete legal names and business addresses, designate contact information for project supervision, specify trade classifications, and list subcontract amounts. Up to 80 percent of contract work may be subcontracted for shelter projects. The document restricts subcontractor changes post-award to specific circumstances including failure to execute contracts at specified prices, lack of appropriate licensing, bankruptcy or insolvency, ineligibility, performance failures, engineer determination of unsatisfactory work, or substantial project delays. The worksheet includes fields to track whether subcontractors are Small Business Enterprise (SBE) or Minority/Women-Owned Business Enterprise (MWBE) certified, with notation of whether changes are additions or deletions and corresponding reason codes.

Subcontractor substitutions designated as additions or deletions require written approval from the City's Procurement & Supply Management office at the specified contact email. SBE subcontractors used to meet percentage requirements must be replaced with other SBE subcontractors unless the prime contractor demonstrates alternative compliance methods. The instructions note that restrictions on subcontracting percentages and multiple-tier subcontractor use do not apply to FDOT LAP or federally funded projects, and only first and second tier subcontractors are permitted unless the contract documents specify otherwise. The worksheet template includes columns for city approver signatures and dates and references January 2020 as the revision date.
