The contractor must document water temperature, pH, and biocide levels with legionella samples and ensure viable samples reach the testing laboratory within required holding times and temperatures. The contractor is responsible for submitting water quality data to the Utah Division of Drinking Water via the Compliance Monitoring Data Portal and providing the Contracting Officer's Representative with chain of custody forms, laboratory data, and written confirmation of timely submissions. Additionally, the contractor must support development of the Annual Drinking Water Quality Consumer Confidence Report (due July 1st) and public notices following any permit violations.

STATEMENT OF WORK

POTABLE WATER TESTING

A. GENERAL:

Contractor is to furnish all labor, supervision, materials, and equipment necessary to accomplish microbial, chemical, and water quality testing of potable water. Service will include testing for legionella, bacteria, disinfection-byproducts, lead & copper analysis, and other water quality testing as required. Water samples will be collected at the George E Wahlen Veterans Medical Center by Contractor staff in bottles provided by the Contractor, then delivered to the lab for testing by the Contractor. All test results will be prepared and emailed to the Contracting Officer’s Representative (COR).

The Salt Lake City VA Medical Center (SLC VAMC) is a permitted community water provider serving fewer than 3,000 customers. The water system ID is: SLC VA Medical Center UTAH18173. The facility purchases water from Salt Lake City Department of Public Utilities and adds a disinfectant to prevent the growth of Legionella in the water distribution system. Due to this additional treatment, the SLC VAMC is required to test for disinfection byproducts (Total Trihalomethanes and Haloacetic Acids), as well as for the presence of total coliform, and conduct lead and copper assessments. Additionally, in accordance with Veterans Health Administration Directive 1061, titled, “Prevention of Health Care-Associated Legionella Disease and Scald Injury from Water Systems,” the facility must also collect water samples throughout the premises for Legionella analysis.

The Contractor is also responsible for submitting the water quality sampling results to the Utah Division Drinking Water via the Compliance Monitoring Data Portal. Data submissions include disinfection-byproducts (DBPs), total coliform, lead and copper. Legionella data will be provided to the COR using the attached template seen in Attachment E.

The C&A requirements do not apply, and a Security Accreditation Package is not required.

B. WORK HOURS:

Normal Work Hours: Samples will be collected and delivered to the lab during normal business hours, typically Mon-Fri 8:00 am-5:00 pm, excluding federal holidays. The eleven holidays observed by the Federal Government are: New Year’s Day, Martin Luther King’s Birthday, President’s Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving Day and Christmas Day, as well as any other day specifically declared by the President of the United States to be a national holiday.

C. SPECIFICATIONS:

1. The Contractor shall assume full responsibility for compliance with all Federal, State, and local laws, rules and regulations governing water quality testing in accordance with the facility’s drinking water permit (SLC VA Medical Center UT18173).

2. The Contractor is required to submit drinking water quality samples (DBP, lead and copper, total coliform) to a Utah certified lab for analysis. See the referenced requirement from the State of Utah [R309-200-8] regarding approved laboratories. Approved Utah certified labs are identified in Attachment J (current as of 2024). Multiple certified labs can be used for different analyses. The Contractor should contact the lab of choice to ensure the accuracy of the information on the spreadsheet.

3. The contractor/contract lab is required to submit water quality data to the COR, and to meet regulatory requirements, the data will also be submitted to the Compliance Monitoring Data Portal. The Compliance Monitoring Data Portal enables labs to upload sample data directly to the Utah Division of Drinking Water. Labs that are “CMDP Certified" are also shown in Attachment J.

4. The Utah certified contract lab will analyze for DBPs (TTHMs and HAA5s) using EPA Methods 524.2 and 552.2. The laboratory’s method detection limit cannot exceed the MCL.

5. The Utah certified contract lab will analyze for the presence/absence of total coliform and perform an E Coli screen (present/absent) using EPA Method 9223B.

6. The Utah certified contract lab will test for lead and copper using EPA Method 200.8. The laboratory’s method detection limit cannot exceed the action levels for lead and copper.

7. The Contractor shall send water samples for Legionella testing to a certified Centers for Disease Control and Prevention (CDC) Environmental Legionella Isolation Techniques Evaluation (ELITE) lab and ensure that the testing of all samples is completed within the time standards established by the CDC and EPA sampling methods. The Contractor must show proof of CDC ELITE certification with their offer. Information about CDC ELITE certified laboratories can be found at https://wwwn.cdc.gov/elite/Public/MemberList.aspx. The Laboratory must also be able to determine and report the Legionella serogroup type.

NOTE: Per CDC guidance, rapid testing methods, such as polymerase chain reaction (PCR) and direct fluorescent antibody (DFA), are not recommended for the detection of Legionella in environmental water samples. In addition to the requirement for current CDC ELITE certification, the laboratory must also have environmental microbiology accreditation by a recognized accrediting program.

8. The Contractor shall furnish all sampling and shipping supplies necessary for the described sampling. The containers shall be of such a design that the water being collected can be filled from the tap.

9. Testing will be accomplished in accordance with the following schedule and quantities:

a. Bacteria (Total Coliform)

i. Monitoring will be conducted in accordance with R309-211

ii. Three (3) tests per month (36 per yr.) as per permit UT18173.

iii. Samples will be collected in buildings 1, 3 and 14. No samples will be collected in restrooms.

iv. A positive total coliform result will require repeat sampling within 24 hours.

v. See Attachment D for the sampling plan and Attachment H for requirements to be followed for any repeat sampling.

b. Disinfection Byproducts

i. Two (2) dual sample sets (TTHMs and HAA5) per quarter (8 per yr.) as per permit UT18173

ii. See Attachment C for the sampling plan.

c. Legionella

i. Collect 111 samples per quarter/444 per yr.

ii. See Attachment A for the sampling plan.

iii. Contractor will document water temperature, pH and level of biocide with each Legionella sample.

iv. All data will be submitted following the WSMT Legionella Reporting Template (Attachment E).

v. The Contractor shall ensure procedures are in place to guarantee viable samples arrive at the CDC ELITE laboratory for testing.

d. Lead & Copper

i. Twenty (20) Tests every three (3) years as per permit UT18173.

ii. Next sample collection timeframe is from June through September 2028 (Option Year 2)

iii. See Attachment B for the sampling plan.

e. Emergency retesting of samples with positive results and/or water quality testing as required: Up to 50 tests per year.

f. All sample collection will be conducted at the beginning of the month to allow for any repeat testing, if necessary, and to meet the regulatory data submittal deadline of the 10th of the following month. The sampling schedule showing the number of samples required each month is provided below.

10. Contractors shall provide personnel to collect all necessary samples at the site in accordance with the prescribed regulatory guidance. The sample collector shall have all the necessary training required to properly collect samples. The VA will provide the Contractor with sampling locations and instructions for how to access sampling locations.

D. REQUIRED REPORTING: The Contractor shall provide the COR with the chain of custody forms and the data from the laboratory. The Contractor is responsible for supplying, completing, and submitting all reports required or requested by permit UT18173, as well as Federal, State, or local ordinances, which pertain to any duties contained in the contract. Additional reporting shall be formatted as directed by the COR. This formatting is necessary to allow the station to easily upload the results to the VA central office via the Water Safety Management Tool (WSMT). The contractor shall submit the water quality sampling results to the Utah Division Drinking Water via the Compliance Monitoring Data Portal. Data submissions include disinfection-byproducts (DBPs), total coliform, lead and copper. The Contractor must provide the COR with written confirmation that the water quality sampling results have been submitted to CMDP on time after each submission event. Legionella data will be provided to the COR using the attached template seen in Attachment E.

E. Consumer Confidence Report: The Contractor will support the development of the Annual Drinking Water Quality Consumer Confidence Report as required by permit UT18173 for submittal by July 1st. The Contractor will assist in incorporating water quality data from the SLC Department of Public Utilities and the water quality samples collected under this contract into the report. F. Public Notices: The contractor will support the development of public notices following any permit violations as described in Public Notice Rule 65 FR25892 (May 2000). The contractor will provide a description of what occurred that resulted in a violation. The Contractor will address all SLC VA comments and make necessary changes to the notices and submit a final version. All public notices will be included in the Consumer Confidence Report. The SLC VA Medical Center will submit the final public notices to the Utah DDW.

G. Shipping: The Contractor shall provide everything required for shipping including all return labels and ice packs for the samples to keep cool in transit. If samples are outside of their holding temperatures or outside of the holding time, it is the sole responsibility of the Contractor. If samples are outside of their holding times or temperatures, it will be recorded in the Quality Assurance Surveillance Plan (QASP), and the Contractor will be required to resample at no additional cost to the government.
