About this file
This is a Standards of Conduct Declaration form (Form AG-010) required by the State of Hawaii for the District Court of the First Circuit in connection with a solicitation for fabrication, printing, and delivery of 50,000 sets of Notice of Entry of Judgment criminal disposition forms. The contractor must declare under penalty of perjury compliance with Hawaii's Code of Ethics (Chapter 84, Hawaii Revised Statutes), including confirmation that the contractor is not a legislator or employee with a controlling interest in a business, has not been personally represented by a former agency employee involved in the contract matter within the preceding two years, and has not engaged legislators or employees for compensation to obtain or perform the contract if such individuals were involved in contract development or award.
The declaration form must be signed by an authorized representative of the contractor and submitted with the contract. If the contractor is identified as a legislator or employee with a controlling interest and the contract value exceeds $10,000, the contract must be awarded through competitive sealed bidding or proposal procedures per Hawaii Revised Statutes Section 103D-302 or 103D-303. Violation of any provision of Hawaii's Code of Ethics renders the contract voidable by the State, and any fees, compensation, gifts, or profits resulting from such violations are subject to recovery by the State. This declaration is a standard compliance requirement for all state contracts in Hawaii.
