About this file
This printing specifications document outlines requirements for the District Court of the First Circuit of the Judiciary, State of Hawaii, to solicit quotes for fabricating, printing, and delivering 50,000 sets of Notice of Entry of Judgment and/or Order forms, commonly known as Criminal Disposition forms. The forms must be delivered in cartons containing 500 sets each, with contractors responsible for all associated labor, equipment, materials, transportation, and delivery costs. Sample forms are available for inspection, and contractors must produce forms identical to the provided samples except for specified wording changes shown in PDF files. The Judiciary emphasizes that the physical sample shall control in the event of any discrepancy between written specifications and the sample provided. No specific response dates, bidder conferences, or award dates are mentioned in this specifications document.
All quoted prices must include labor charges, equipment, supplies, taxes, and any other costs necessary to complete the work. The six-part forms must be constructed from carbonless paper in specified colors (green, pink, white, yellow, blue, and white) with corresponding weights, printed with black ink on face sheets and screened text on certain backs, with red ink used for marginal designations. White, pink, and yellow sheets must contain a minimum of 30% post-consumer recovered material, with contractors required to submit a completed SPO Form-8 Certification of Recycled Content. At the time quotations are due, offerors must be compliant with the Hawaii Compliance Express system, and all submissions must be made through the State of Hawaii eProcurement System (HIePRO). The contractor must deliver the exact quantity with no overages or shortages, with the Judiciary refusing payment for any excess forms delivered. Delivery must occur during specified hours (9:30 a.m. to 11:30 a.m. and 1:30 p.m. to 3:30 p.m. Hawaii Standard Time, Monday through Friday, excluding state holidays) to 1111 Alakea Street, 1st Floor, Honolulu, Hawaii 96813. Payment shall be made upon certification of satisfactory delivery, and the Officer-in-Charge, Erica Uahinui, can be contacted at 808-538-5127 or erica.b.uahinui@courts.hawaii.gov for inquiries and pre-delivery instructions.
