1. General: The Offeror shall submit present and past performance information for itself and any joint venture member in accordance with the format contained in the instructions contained in Section L and the following paragraphs. The evaluation of the offeror’s/joint venture member’s present/past performance WILL NOT include the present/past performance of any of the JV’s subcontractor(s)/vendor(s)/supplier(s). A joint venture is defined as a contractual agreement joining together two or more parties to undertake a particular business transaction or project and is intended to exist for a limited time period. The individual entities may retain their individuality and operate under a joint venture agreement, or the joint venture creates a separate corporation, limited liability company, or partnership.

2. Past Performance Format: The requested present and past performance information shall be provided in a separate volume labeled "Volume III - Past Performance." Each offeror/joint venture member shall utilize the Past Performance Summary (PPS) to complete information for each active or completed contract in the past three (3) years that the offeror/joint venture member considers relevant in demonstrating its ability to perform the proposed effort. If the total number of such contracts exceeds two (2), each offeror/joint venture member shall address its two (2) most recent and relevant contracts.

3. Number of Contract Submissions: With respect to the limit of two (2) contracts specified in the paragraph above, each one-time, fixed-quantity, standalone contract is considered 1 contract.

For long term contracts that provide for option exercise for additional supplies/services beyond the initial or basic award or the placement of multiple orders (e.g. IDIQ or requirements contracts as defined in FAR 16.5, basic ordering agreements, blanket purchase agreements, or other long term arrangements) performance under the basic contract and any options or performance of a series of orders issued against the same contract may be submitted as 1 contract subject to all of the following conditions:

a. Performance is relatively continuous (e.g. production/performance breaks do not exceed 6 months)

b. The nature of performance is related.

c. All of the submitted performance satisfies the past performance recency as stated above and relevance criteria set forth in the table below:

Adjectival Rating Description Very Relevant Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires.

Contract valued ≥ $10M Assembling and delivering kits with multiple parts and complex technical specifications that support DoD or other US Government weapon systems Manufacturing/producing cables and wiring harnesses

Relevant Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires.

Contract valued from ≥ $5M - < $10M Assembly and delivery of kits, cables, and/or wiring harnesses that support

DoD or other US Government weapon systems Somewhat Relevant Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires.

Contract valued from ≥ $2M - < $5M.

Assembly and delivery of kits, cables, and/or wiring harnesses that support

Contract valued at < $2M No experience in assembling and delivering kits, cables, and/or wiring harnesses that support DoD or other US Government weapon systems

When submitting an individual order (or series of orders) under an ordering type contractual vehicle, it is important that the offeror's past performance information be accurate, complete, and easily understood. If the information is not submitted in the correct format, the Government will not be able to evaluate the effort for recency, relevancy, and quality.

4. Past Performance Summary (PPS): The offeror/joint venture member and/or critical subcontractor shall focus its PPS responses so that there is a clear correlation between present and past performance and the requirements of this RFP. b. Program/Project Manager, or equivalent

c. Administrative Contracting Officer/Contract Administrator or equivalent

d. Other (As specified in your PPS)

If problems were encountered during the performance of the identified contracts, provide evidence of the ability to isolate the root causes of problems and include in the PPS a description of programs or actions taken to resolve those causes. Problems not addressed in the PPS but found by the Government during the evaluation of the information in this volume, will be assumed to still exist.

Note: In the case of Contractor Performance Assessment Report System (CPARS), if your input has already been provided and the rationale/circumstances have not changed, DO NOT repeat them here.

5. Present/Past Performance Questionnaires (PPQs): Offerors shall send PPQs to the customers of contracts submitted on their PPS’. PPQs shall be sent directly to the Government at DLAHSVPMContracts@dla.mil by the time and date of the RFP closing listed in Section A.

Proposed unit/CLIN prices shall be expressed in whole dollars (no cents). Data beyond that required by this instruction shall not be submitted. All data relating to the proposed price, including all required supporting documentation, must be included in the section of the proposal designated as the Price volume. Under no circumstances shall this data and documentation be included elsewhere in the proposal.
