Format: USG requires contractor response digitally and in the English language. Contractor format is acceptable. With the exception of the Round x Round data in paragraph 1 which shall be filled in per the template provided, see Attachment F.

Blocks 10-13: The report is to be submitted 30 days after contract award.

The report shall include at a minimum the below information.

1. Provide details on what type of testing your system conducted in the Developmental phase (pre-production), Soldier test, and Production. Provide a copy of your Test Duty Cycle or Operation Mode Summary/Mission Profile (OMS/MP) (i.e. terrain breakdown, rounds (charge/range), km per day etc) utilized during testing. Provide detailed firing record Round x Round during testing (see Attachment F, PM SPHS Round by Round Template_final)).

2. Describe your process for documenting and categorizing incidents/failures that occurred during system level testing. Provide an example of incident reports detailing failures that prevent mission completion that occurred during system level testing.

3. Describe the classification of failures and severity of loss of function. Do you utilize a Failure Definition Scoring Criteria (FDSC) guide used to outline and classify severity of failures found during system level tests. Example of an FDSC guide is NATO Standard Allied Dependability Management Publication-03 (ADMP) Guidance For Classification and Analysis of Dependability Events. Provide a copy of the document you utilized.

4. Provide the most up to date system and subsystem reliability predictions and allocations and identify the last time the data was updated.

5. Describe how a system and subsystem Design Failure Mode Effects Analysis (DFMEA) is maintained.

Provide the DFMEA for the Armament subsystem.

6. Provide the Failure Report Analysis and Corrective Action System (FRACAS) process (or referred to as a closed loop corrective action process), and when it is utilized.

7. Describe how System Reliability Data, such as Mean Time Between Failure (MTBF), Mean Miles Between Failure (MMBF), Mean Round Between Failure (MRBF)) and Maintainability data (Maintenance Ratio (MR)/Mean Time To Repair (MTTR) from conducted tests is determined. Provide current Reliability Data estimates.

Blocks 10-13: Contractor shall submit a draft CDRL 90 days after contract award. USG will conduct a review within 15 days after receipt. Contractor shall propose a time for a four hour meeting to occur after the 15 day USG review period. The intent of the meeting will be to review any USG comments to the draft proposal. The final submission is due from the contractor 10 days after the review meeting.

The report shall include the following information, considering the three threats types previously listed:

a. Design description of the material (armor, energy attenuation, etc.) that protects the crew compartment and associated protection levels (also known as "force protection")

b. Design description of the material (armor, energy attenuation, redundancy, etc.) that protects the critical subsystems and associated protection levels (also known as "survivability"). Critical subsystems are those subsystems necessary to accomplish the primary mission of the vehicle (move, shoot, communicate).

c. A summary of ballistic testing completed to date without results. The intent of this request is to understand what test results exist and for what generic types of threats. Include information available for both coupon testing and system level testing, if available.

d. Describe if armor is integral to the system design or if it is provided in the form of a kit ("add-on armor").

The contractor may reference North Atlantic Treaty Organization Standardization Agreement 4569 (NATO STANAG 4569) for applicable protection ratings and methods.

The report must include, at a minimum, the information outlined below.

a. Describe the braking system design, the vehicles stopping performance, and the standards used for design and evaluation of the braking system.

b. Provide usable fuel capacity and allowable fuel types. Usable is defined as total volume less the volume that is unavailable while the vehicle is on 60 percent, or maximum allowable, grade.

c. Provide fuel consumption information and describe influence of fuel types.

d. Provide a description of the vehicles self-recovery features, such as ability and methods to complete like-vehicle towing, recovery provisions and their rated capabilities, winches, vehicle preparation for towing, methods for recovering vehicles with damaged running gear (run flats, "short-tracking," etc.).

e. Describe vehicle characteristics in accordance with Mobility Analysis Tool-Computer Edition (MATCE) Input Datasheet Tracked Vehicles or MATCE Input Datasheet Wheeled Vehicles as applicable to the vehicle being offered for evaluation. The USG will provide the MATCE Data Sheets as Attachments C and D.

f. For wheeled vehicles only, provide an overview of tire sizes, use of adjustable inflation pressures, ground pressure, and an overview of the driveline redundancy during battle damage.

g. Identify the type and quality of on-board electrical power for both hardware integration and soldier use.

h. Describe the amount and type of energy storage (e.g. lithium-ion batteries, make/model, 4x1.5 kw-hrs) utilized on the platform.

a. The Contractor shall verify the Product Level Technical Data Package (TDP) consists of elements necessary to provide the design, engineering, manufacturing, inspection, packaging, and quality assurance provisions necessary to enable the procurement or manufacture of an item. See MIL-STD-31000B (Distribution A) or Data Item Description (DID) number: DI-SESS-82238A (Distribution A) for complete Product Level TDP definition.

b. The Contractor shall provide all applicable specifications and standards in which the TDP is derived from. (e.g. Internationally accepted standards/specification, Government standards/specifications, Industry standards, for hardware, cables, schematics, and software.)

c. The Contractor shall provide the format in which the TDP is derived from and maintained. (e.g. 2-dimensional drawings, 3-dimensional CAD models with applicable software)

d. The Contractor shall provide all applicable Configuration Management Standards used to manage the Technical Data Package for both hardware and software.

e. The Contractor shall provide the Level of Intellectual Property Rights for the Technical Data Package.

The report shall include, at a minimum, the following information:

a. Please describe how many levels of maintenance within a military support structure are required to support this vehicle (for example: the US Army utilizes two level of maintenance, Field level and Sustainment level).

b. An assessment of basic logistical support that would be required to keep the system operationally available (personnel, tools, spare parts stockage, training, training devices, facilities, documentation, other vehicles, etc.).

c. Examples of how the Technical Data Package is formatted and what it includes.

d. Key examples of common Operation, Maintenance, and Repair Parts Technical Instructions.

e. Identify any specific standards you have used to provision vehicle components and spare parts (e.g., Society of Automotive/Aerospace Engineers - Government Electronics and Information Technology Association Standard (SAE-GEIA-STD-0007) or similar International Organization for Standardization (ISO) provisioning instructions). Please provide the provisioning standard(s) and one selected installation and assembly level provisioning breakdown of components. (e.g.: provisioning for Cannon, Powertrain, Loading and Ramming Installations and assemblies).

f. An assessment of common repair parts stock availability.

g. Any required training, education, or skill set requirements to operate or maintain the system.

h. Any required training aids, devices or simulators used to train on, operate and maintain the system.

i. A complete, descriptive list of tools, sets, and kits, required to operate and maintain the system.

j. The standard list of recurring, preventative maintenance tasks, services, and checks required to keep the system at an operationally usable level. (e.g., monthly, semi-annual, annual, before and after usage, etc.)

k. A complete descriptive list of any on-board items (tools, diagnostic devices, spare parts, etc.) that are required to keep the system operationally available.

l. A complete descriptive list and narrative of any components on the system and in stock supply that require special long term storage requirements, including containers.

Definitions Resupply Vehicle/System (RSV) Self-Propelled Howitzer (SPH)

The report must include, at a minimum, the information outlined below.

1. Provide the interior storage volume (cu ft.).

2. Provide the maximum payload of the RSV.

3. Describe the storage capacity for:

a. Conventional Projectiles

b. Propellant Charges (with/without container)

c. Precision Projectiles (with/without container)

4. Describe the storage capacity for both vertical and horizontal projectile storage.

5. Describe how fuzes (also describe quantity), primers (also describe quantity), and miscellaneous supplies and equipment are stored within the RSV.

6. Provide the crew capacity of the RSV.

7. Provide a description of any monitoring devices or sensors within the RSV.

8. If the RSV utilizes a modular container, describe limitations of loading and unloading the container/ module on and off the vehicle. Also describe any special tooling or equipment necessary for loading/ unloading.

9. If the RSV utilize a modular container, provide a description of any available onboard power or pneumatics.

10. Describe the amount of time to fully combat load the RSV from empty. Provide the number of personnel required to fulfill this task.

11. Describe the amount of time the RSV can rearm the SPH to full combat load from empty. Provide the number of personnel required to fulfill this task.

12. Describe the required time for the RSV to go from a transport (driving) condition to a rearm (ready to rearm the SPH) condition, and vice versa.

The Cannon & Ammo Handling Safety Summary shall contain the following requirements:

1. The contractor shall develop a cannon safety summary that outlines the safety activities conducted on the cannon assembly to be utilized in the shoot off howitzer. The safety summary shall include:

a. The safety test conducted on the cannon (reference appropriate military standards, international test operating procedures [ITOPs], or North Atlantic Treaty Organization Standardization Agreement/Allied Ordnance Publications STANAG/AOP])

b. The ammunition utilized in the safety tests.

c. The canon tube wear, canon tube fatigue, and canon tube safe service life.

d. The permissible maximum pressure (PMP), the cannon design pressure (CDP), and the proof pressure

(PP).

e. Evidence that the cannon has been proofed.

2. The contractor shall provide an overview drawing of the cannon assembly that includes the following data:

a. Overall Length of Cannon Tube

b. Maximum and minimum chamber inside diameters

c. Chamber volume with the breech closed. Include the reference projectile and the projectile seating distance from the rear of the projectile to the breech end of the cannon tube.

d. Length from breech face of the cannon tube to the commencement of full rifling.

e. “Length from the propellant retention feature to the commencement of full rifling.

f. Length from the commencement of full rifling to the muzzle end of the cannon tube.

g. Primer interface dimensions

h. Primer location relative to centerline of Cannon tube j . Number of rifling grooves.

k. Bore diameter including tolerances.

l. Twist of rifling to include the direction and rate of turn.

m. Length that chrome is applied to the bore measured from the breech end of the cannon tube.

n. Standoff length from the closed breech face to the propellant charge retention feature.

1. The contractor shall provide detailed crew drills for each of the following operations:

a. Emplacement

2. The contractor shall specify what the current U.S equivalent Military Occupational Specialty (MOS) is required for each crew member.

3. The Critical Crew Drill Report shall be developed using the notional U.S equivalent MOS for each crew member.

4. For each Critical Crew Drill operation, the contractor shall detail what each crew member duty/tasks are along with a timeline.

5. For each Critical Crew Drill operation, the contractor shall provide a task analysis that lists the equipment/human factors interfaces that each crew member interacts with when performing each task as well as the sensory, cognitive, and motor demands that the tasks impart on the users, who operate and maintain (i.e., troubleshoot) the system and its components. This shall include operator/crew supervision of automated functions. Critical Task Analysis Report (DI-HFAC-81399B) and the Human Engineering Systems Analysis Report (DI-HFAC-80745C) are available as guidance.

6. The Critical Crew Drill Report shall provide basic dimensions, locations, and human factors requirements (e.g. hand wheel torques) for each of the crew member interfaces. The contractor may use the Human Engineering Design Approach Document - Operator (HEDAD-O) (DI-HFAC-80746C) as guidance.

m. Describe degrade operations to include any manual backups for conducting a mission. The report shall include at a minimum:

1. References. A list of source materials used in preparing the report. Include for example, government and contractor reports, standards, criteria, technical manuals and specifications. If references are numerous, put them in a bibliography as an appendix.

2. A system description. A brief identification of the system and its purpose. Address significant health hazard issues that are identified later in the report.

3. A background. A description of the system and its intended operation. Include pertinent components or subsystems which contribute most to a health hazard, The identity of the intended users and the type of protective clothing and equipment, if any, available to the user. A summary of the evaluations or assessments performed on system prototypes or developmental models.

4. Identification of health hazards issues. A description and discussion of each potential or actual health hazard issue of concern for each subsystem or component. A health hazard is an existing or likely condition, inherent to the operation, maintenance, transport or use of materiel that can cause death, injury, acute or chronic illness, disability, or reduced job performance of personnel by exposure to physiological stresses.

a. A system breakout. Use sub-paragraphs for each subsystem or component, with additional sub-paragraphs for each health hazard discussion. Include sufficient detail to clearly define the specific problem, issued involved and reasoning behind the analyses.

b. Material information. For each proposed and alternative material, include the following:

(1) Material identification. Include material identity; common or trade name; chemical name; chemical abstract service number; national stock number (NSN) or local stock number; physical form (solid, liquid, gas); and manufacturers and suppliers.

(2) Material use and quantity. Include component name, description, and code, and/or operations details for the material. Total system and program, life-cycle quantities to be used. For mixtures, concentrations for each ingredient.

(3) Hazard identification. The detrimental effects of the material on the system, personnel, environment, or facilities. Specific health hazards are: chemical hazards, physical hazards (noise, heat/cold stress, etc.), biological hazards, ergonomic hazards, ionizing and nonionizing radiation hazards, and other hazards that may be formed by the test, operation, SFAE-GCS-HR 1

3. A system operations section. This section shall include:

a. A description or reference of the procedures for operating, testing, and maintaining the system. Discuss the safety design features and controls incorporated into the system as they relate to the operating procedures.

b. A description of any special safety procedures needed to assure safe operations, test, and maintenance, including emergency procedures.

c. A description of anticipated operating environments, and any specific skills required for safe operation, test, maintenance, transportation, or disposal.

d. A description of any special facility requirements or personal equipment to support the system.

4. A system safety engineering section. This section shall include:

a. A summary or reference of the safety criteria and methodology used to classify and rank hazardous conditions.

(1) Safety risk assessment criteria shall follow MIL-STD-882E, Change 1.

5. A conclusions and recommendations section. This section shall include:

a. A short assessment of the results of the safety program efforts. A list of all significant hazards along with specific safety recommendations or precautions required to ensure the safety of personnel and property. The list of hazards will be categorized as to whether or not they may be expected under normal or abnormal operating conditions.

b. For all hazardous materials generated by or used in the system:

(1) Materiel identification as to type, quantity, and potential hazards.

(2) Safety precautions and procedures necessary during use, storage, transportation, and disposal.

(3) A copy of the Safety Data Sheets (SDSs) as required.

c. A statement that the system does not contain or generate hazardous materials (i.e., explosive, toxic, radioactive, carcinogenic, etc.).

d. An attachment of all Final (if applicable) or Interim Hazard Classifications (FHCs/IHCs) generated for Ammunition and Explosive (A&E) materials and components:

Note: An IHC is required, at a minimum, for all ammunition items provided by the contractor.

(1) FHCs and IHCs shall be generated in accordance with TB 700-2 and AOP-7.

(2) FHCs and IHCS shall demonstrate that propellant sustainability testing was performed as part of the FHC/IHC procedure.

e. A statement signed by the contractor system safety manager and the program manager stating that all identified hazards have been eliminated or controlled and that the system is ready to test, operate, or proceed to the next acquisition phase. In addition, include recommendations applicable to the safe interface of this system with the other system(s).

6. A references section. This section shall include a list of all pertinent references, such as test reports, preliminary operating manuals and maintenance manuals.

Blocks 10-13: Contractor shall submit a draft CDRL 90 days after contract award. USG will conduct a review within 15 days after receipt. The final submission is due from the contractor 10 days after the review meeting.

The report shall include at a minimum:

1. A system description section. This will consist of summary descriptions of the physical and functional characteristics of the system and its components. Reference to more detailed system and component descriptions, including specifications and detailed review documentation shall be supplied when such documentation is available. The capabilities, limitations and interdependence of these components shall be expressed in terms relevant to safety. The system and components shall be addressed in relation to its mission and its operational environment. System block diagrams or functional flow diagrams may be used to clarify system descriptions. Software and its role(s) shall be included in this description.

2. A data section. This will consist of summaries of data used to determine the safety aspects of design features.

3. A hazard analysis results section. This will consist of a summary or a total listing of the results of hazard analysis. The following are the content and format requirements:

a. A summary of the results.

b. A listing of identified hazards, in narrative or matrix (sometimes called columnar or tabular) format, to include the following information:

(1) System/subsystem/unit. The particular part of the system that this analysis is concerned with. For example, if this item(s) applies to a radar system modulator, use “modulator.” If there are several modulators in the system, clearly specify which one the analysis pertains to.

(2) Component(s) failure mode(s). All component failure modes which can result in a hazard. Failure modes generally answer the question of “how” it fails.

(3) Subsystem failure mode(s). The subsystem failure mode descriptions for the system hazard analysis (SHA) are similar to the component descriptions provided in the subsystem hazard analysis (SSHA). However, emphasis is now placed on failure affecting interfacing subsystem operations.

(4) System component/phase. The particular phase/component that the analysis is concerned with. This could be a system, subsystem, component, software, operating/maintenance procedure or environmental condition.

(5) System event(s) phase. The configuration of phase of the mission the system is in when the hazard is encountered, for example, during maintenance, during flight, during pre-flight, full-power applied, etc., or it could be encountered in all system events.

(6) System operation description. A description of what is normally expected to occur as the result of operating the component/subsystem or performing the operating/maintenance action.

(7) Hazard description.

(a) A brief description of the hazard of hazardous material, for example, “Radiation leakage from radar set waveguide.”

(b) A complete description of the potential/actual hazards inherent in the item being analyzed, or resulting from normal actions or equipment failure, or handling of hazardous materials.

(8) Hazard identification/indication. A description of operator/crew indications which include all means of identifying the hazard to operational/maintenance personnel.

(9) Effect of hazard. The detrimental effects which could be inflected on the subsystem, system, other equipment, facilities or personnel, resulting from this hazard. Possible upstream and downstream effects shall also be described.

(10) Risk assessment. A risk assessment for each hazard (classification of severity and probability of occurrence). This is the assessment of the risk prior to taking any action to eliminate or control the hazard.

(a) Risk assessment criteria shall be in accordance with MIL-STD-882E, Change 1.

(11) Recommended action. The recommended action required to eliminate or control the hazard. Sufficient technical detail is required in order to permit the design engineers and the customer to adequately develop and assess design criteria resulting from the analysis. Include alternative designs and life cycle cost impact where appropriate.

(12) Effect of recommended action. The effect of the recommended action on the assigned risk assessment. This is the risk assessment after taking action to eliminate or control each hazard. If the recommended action will result in cost/schedule/performance penalties to the extent that the contractor requires government approval prior to incorporation, then these considerations shall be addressed.

(13) Remarks. Any information relating to the hazard not covered in other blocks, for example, applicable documents, previous failure data on similar systems, or administrative directions.

(14) Status. The status of actions to implement the recommended, or other, hazard controls. The status shall include not only an indication of "open" or "closed," but also reference to the drawing(s), specification(s), procedure(s), etc., that support closure of the particular hazard.

(15) Caution and warning notes. A complete list of warnings, cautions, and procedures required in operating and maintenance manuals and for training courses.

Format: USG requires contractor response digitally and in the English language. Contractor format is acceptable.

Blocks 10-13: Contractor shall submit a draft CDRL 90 days after contract award. USG will conduct a review within 15 days after receipt. Contractor shall propose a time for a four hour meeting to occur after the 15 day USG review period. The intent of the meeting will be to review any USG comments to the draft proposal. The final submission is due from the contractor 10 days after the review meeting.

The report shall detail the system's ability to include supporting documentation to meet each of the 10 Cyber Survivability Attributes identified in the Cyber Survivability Endorsement (CSE). The USG will provide the CSE as Attachment E.

Contractor shall propose a time for a four hour meeting to occur after the 15 day USG review period. The intent of the meeting will be to review any USG comments to the draft proposal. The final submission is due from the contractor 10 days after the review meeting.

The report must include, at a minimum, the following information:

a. Describe the environmental testing standard that has been used for evaluating the vehicle to include requirements that have been verified.

b. Provide an environmental assessment to include completed and on-going environmental testing at vehicle to include powertrain, ammunition handling, recoil, breech and cannon. "Department of Defense Test Method Standard Environmental Engineering Considerations and Laboratory Testing MIL-STD-810 Revision H w/Change1, 18 May 2022" and "ITOP-3-2-506(1) - Artillery (Self-Propelled and Towed), Final International Test Operations Procedure" may be referenced in your response.

c. Provide an assessment that includes climatic conditions related to high and low temperature, temperature shock, rain, humidity, fungus, salt fog and corrosive environments, and sand and dust.

d. Provide an assessment that includes induced climatic conditions related to transportation modes for sea, road and rail.

e. Describe any system level Electro Magnetic Interference testing that has been completed. "MIL- STD-464 - Electromagnetic Environmental Effects Requirements for Systems" may be referenced in your response.
