About this file

This is a Past Performance Questionnaire template used to evaluate contractor performance on completed federal contracts. The document consists of ten pages and is designed to be completed by the contracting officer and submitted to references for direct completion and return to the Government.

The questionnaire is divided into two main sections: contractor information and evaluation questions. The contractor information section (Blocks 1-4) requires the offeror to provide company details including name, CAGE code, DUNS number, contact information, work role (prime contractor, subcontractor, or joint venture), contract number, contract type (firm fixed price, cost reimbursement, or other), contract title, location, award and completion dates, original and final contract prices, complexity level (high, medium, or routine), and relevance to current requirements. The client information section (Blocks 5-8) is completed by the reference and includes their name, title, phone, email, their role in the project, questionnaire completion date, and signature. The reference must submit the completed form directly to the Government contact provided.

The evaluation questions (29 total) assess contractor performance across five categories: Schedule (4 questions on responsiveness and timeliness); Management of Key Personnel (2 questions on preparation, training, and subordinate management); Quality of Services (13 questions addressing technical expertise, compliance, quality control, disputes, terminations, and equipment); Business Relations (6 questions on subcontract management, labor disputes, legal compliance, customer service, and cooperation); and Overall Performance (4 summary questions on satisfaction and future contract award likelihood). Questions require yes/no/N/A responses, with narrative explanations required for negative answers. The form includes space for remarks and listing of additional contractor references.
