Continuation of Description

PERFORMANCE WORK STATEMENT (PWS)

Part 1: Introduction

Introduction: The Contractor will administer the United States Army Reserve (USAR) Psychological Health Program (PHP). The services provided will be of nature and scope described in the paragraphs below. The PHP is designed to promote mental fitness and personal wellness to support operational readiness through linkage to behavioral health treatment, consultation, critical incident response, and clinical case management (CCM). The United States Army Reserve (USAR), also known as the Reserve Component (RC), currently has approximately 200,000 Soldiers. The primary mission of the RC is to provide trained and ready units and individuals to mobilize and deploy in support of the national military strategy. To fulfil this mission, Soldiers are required to maintain a constant state of physical and psychological readiness. The RC provides a variety of support services for Soldiers and their dependents, also referred to as family members (FM), but the RC has no mechanism for providing treatment. When on active duty, Soldiers and their dependents are eligible for treatment through the Military Health System, to include Military Treatment Facilities (MTFs). When not on active duty, Soldiers seek treatment through the Veterans Administration (VA) if eligible, or through civilian health care systems in their communities. The USAR Phycological Health Program is a support service that will assist Soldiers and dependents to access the services required to maintain optimum mental fitness and readiness.

General Information

1. General: This is a non-personal service(s) contract under which the personnel rendering the service(s) are not subject, either by the contract's terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the Government and its employees. The contractor shall provide the following Contract Service Providers (CSP(s)): 17 Regional Directors Psychological Health (RDPH), 8 Nurse Case Managers (NCMs) and 2 Administrative Assistants (AAs) to accomplish performance of work under this contract within the Army's Reserve Surgeon Directorate Psychological Health Program.

1.1. Description of Services: This is a non-personal service(s) contract to provide equipment, supplies, and services in accordance with this contract not provided for by the Government under this PWS. The Contractor may choose to use its own resources in lieu of Government resources at no additional cost to the Government. All Contractor property shall be clearly marked to identify it as Contractor property. The Contractor shall provide identification badges for its employees. These badges shall depict the employee's name, photograph, and Contractor's name, as a minimum. The Contractor shall provide each professional office with all general supplies and Government approved training, promotional materials, and stationery necessary for program operations, in fulfillment of this contract.

Specific Government organization logos or other insignia will be provided by the COR. Under no circumstance may the Contractor's corporate name or logo appear on any material used, unless pre-authorized by the COR.

1.2. Background: The United States Army Reserve, a Reserve Component of the U.S Department of Defense (DOD), is required to provide mental health support services in accordance with Section 723 of the National Defense Authorization Act for FY 2007 (Public Law 109-163), and the 2007 Report of the DOD Task Force on Mental Health. Department of Defense Instruction (DODI), 6490.09, DOD Psychological Health Program, mandates the appointment of Psychological Health Coordinator. The DODI also outlines a uniform Psychological Health leadership structure for the Reserve Component (RC) that parallels the Active Component (AC) structure, to ensure that the psychological health needs of Reservists and National Guard (ARNG) members and their families are met. 10 U.S.C., Chapter 55 establishes: federal responsibility for Prevention, Mitigation, and Treatment of Blast Injuries (Pub. L. 109-163 (1) (D); Access to Health Care Services for Beneficiaries Eligible for

TRICARE and the Department of Veterans Affairs Health Care (Pub. L. 107-314, (A), (C); Health Care Management Demonstration Program (Pub. L. 106-398, (a), (b, 1-2), specifically to improve, develop and maintain intervention and rehabilitation programs for returning troops who have developed post- traumatic stress disorder (PTSD), experienced traumatic brain injuries (TBI), and /or other behavioral health problems associated with an individual's inability to reintegrate into civilian life. Some states and local communities currently lack access, coordination and/or the professional intervention services of mental health providers and related support systems. The RC faces unique challenges to provide mental health support to its Soldiers. Reserve Centers are community based and are seldom within the catchment area of a military treatment facility. Since the RC does not provide treatment, all treatment is provided externally through the active component Military Health System, TRICARE, the VA, civilian health insurance (to include state health insurance), and personal finances. This contract addresses the recommendations of the Department of Defense Task Force on Mental

Health, June 2007; Recommendation 5.4.1.15: The Assistant Secretary of Defense for Reserve Affairs should appoint a Director of Psychological Health (DPH) who serves as a member of the DOD Psychological Health Council. This Director's duties should parallel the duties of the Active- Duty Service Directors of Psychological Health and Recommendation 5.4.1.16, Each Service Reserve Component should appoint a full-time Director of Psychological Health to the staff of the United States Army Reserve Command (USARC) Surgeon. Where Reservists are organized by region, a full-time Regional Psychological Health Director should be appointed. This contract also complies with the subsequent mandate of DODI 6490.09, February 2012; 5b, which states: Each Service RC shall appoint at least one full-time RDPH to the staff of the USARC Surgeon. Where Reservists are organized by region a full-time regional DPH shall be appointed. Services are to be part of a comprehensive overall effort to support Soldier readiness and assist Soldiers to better cope with the sequelae of TBI and/or post-traumatic stress. They are to decrease the stigma associated with seeking mental health services as well as improve access to qualified mental health providers at the critical point, when a RC Soldier or dependent recognizes the need for mental health intervention. The purpose of this contract is to provide psychological health support services, designed for effective delivery within the RC structure and culture. The Contractor will identify and provide Regional Directors Psychological Health (RDPH) and Nurse Case Managers (NCMs) by region (Readiness Division as of Oct 2017), and at additional locations as identified by attachments to this document. Services will be delivered wherever RC Soldiers and their dependents are located, to include all US territories. RDPHs and NCMs will develop partnerships with other RC support services, with a goal of ensuring comprehensive coverage without duplication of services.

1.3. Scope: The Contractor shall provide non-personal service(s). Under this contract the Contractor shall support the PHP requirements of the USAR. In the performance of this contract, the Contractor, independently and not as an agent of the RC, shall provide all necessary personnel, management, and supervision to support comprehensive psychological health support services. The goals of the service are to provide high quality services that are SM specific, friendly and comprehensive, while typically increasing utilization rates to promote SM readiness for deployment, provide assessment, short-term problem resolution counseling, referral services, provide clinical case management and provide Traumatic Event Management, provide education, training, and support.

1.4. Objectives: The objective of this contract is to provide customer service. Customer satisfaction is the most significant external indicator of the success and effectiveness of all services provided and can be measured through customer complaints. Also, to reduce or eliminate the incidence of waste, fraud, and abuse to government financial systems.

1.5. General Information:

1.5.1. Quality Control Plan (QCP): The Contractor shall develop and maintain a QCP to ensure services are performed in accordance with (IAW) this PWS. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The Contractor's QCP is the means by which it assures that the work provided complies with the requirements of the contract.

The Contractor's Proposed QCP shall be submitted to the Contracting Officer (KO) through the Contracting Officer's Representative (COR) for review within 15 days after date of contract award.

The Government will review and either notify the Contractor in writing of acceptance of the plan or return their comments to the Contractor within 15 days. If the Government has provided comments, the Contractor shall then have 10 days to submit a Final QCP. After receipt of the Final QCP, the Contractor may receive the Contracting Officer's acceptance in writing. Any proposed changes to the accepted QCP are required to be resubmitted for acceptance by the Contracting Officer no later than 30 days prior to the anticipated change and before implementation by the Contractor. The timeline noted above will apply for review and acceptance for proposed changes.

At a minimum, the QCP must include and answer the following to be acceptable:

(a) A chart showing the organizational structure and lines of authority, the names, qualifications, duties, responsibilities, and classification of each member of the Contractor's Quality Control Team;

(b) How the Contractor will monitor work to ensure performance complies with all deliverables (etc.

timelines, deadlines, and goals);

(c) How the Contractor will monitor work to ensure performance complies with all specifications and requirements of the contract, including the contract's clauses;

(d) How the Contractor will monitor and ensure staff qualifications remain current and valid including Department of Defense (DoD) Contractor Personnel Office (DOCPER) processes /approvals throughout contract performance;

(e) How the Contractor will ensure all keys issued will remain controlled items (Paragraph 1.7 Key Control);

(f) How the Contractor will inventory and track maintenance of all Government Provided Equipment / Materials;

(g) How the Contractor will identify, investigate, and correct any non-conforming performance and prevent similar deficiencies in the future; and

(h) How the Contractor will file and save all Quality Control related documents for the life of the contract plus 5 years.

1.5.1.1. RESERVED

1.5.1.2. The contractor is responsible for the quality of all work performed. The contractor measures that quality through the contractor's own quality control (QC) program. QC is work output, not workers, and therefore includes all work performed under this contract regardless of whether the work is performed by contractor employees or by subcontractors. The contractor's QCP will set forth the staffing and procedures for self-inspecting the quality, timeliness, responsiveness, customer satisfaction, and other performance requirements in PWS. The contractor will develop and implement a performance management system with processes to assess and report its performance to the designated government representative. The contractor's QCP will set forth the staffing and procedures for self-inspecting the quality, timeliness, responsiveness, customer satisfaction, and other performance requirements in PWS. This QASP enables the government to take advantage of the contractor's QC program.

1.5.2. Quality Assurance: The Government will evaluate the Contractor's performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards. .
