About this file
This is a Past Experience Information Form (Attachment 3) used as an evaluation factor for the FAA's Print Consumables Indefinite Delivery, Indefinite Quantity (IDIQ) contract solicitation (697DCK-25-R-00368). The form is a template requiring vendors to document their relevant past experience with printer consumable sales and distribution. Vendors must provide prime contractor and subcontractor details, customer information, period of performance dates, yearly commercial value of printer consumable sales, brand variety handled, and overall contract value for each experience example submitted.
The form requires vendors to submit a narrative (maximum two pages) demonstrating that their past experience meets the size, scope, and complexity requirements outlined in the solicitation. The narrative must address four mandatory evaluation criteria: (i) Technical performance, including compliance with contract requirements and accuracy of orders and deliverables; (ii) Schedule/Timeliness, demonstrating on-time delivery and reliable order processing; (iii) Customer Satisfaction, documenting customer feedback on service and product quality; and (iv) Cost Control, detailing past cost management efforts. This form is part of the FAA Office of Information and Technology's evaluation process for awarding a five-year IDIQ contract (one base year plus four one-year ordering periods) to supply 750+ items of toner and printer consumables, reserved for Small Businesses under NAICS code 325992 with a 1500-employee size standard.
