About this file

This document is a Subcontractor Summary Worksheet template issued by the City of St. Petersburg, Florida, designed for bidders on a shelter project to disclose all intended subcontractors as part of their bid submission. The worksheet requires prime contractors to identify themselves, provide the name and date of the employee completing the form, and list comprehensive information for each subcontractor including legal name, primary business address, project supervision contact details, tier classification, trade classifications, and subcontract amounts. The document specifies that up to 80 percent of the shelter contract work may be subcontracted, with restrictions limiting subcontractors to first and second tier levels unless otherwise noted in contract documents. Subcontractors cannot be changed or substituted after award and the pre-construction meeting except under seven specified circumstances, including failure to execute a contract, lack of proper licensing, insolvency, ineligibility, performance failures, unsatisfactory work quality, or substantial project delays.

The worksheet establishes that Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substitution becomes necessary, unless the prime contractor demonstrates SBE requirements are being met through alternative means. Requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management office, specifically to the Contracts Compliance Manager. The document notes that restrictions on subcontracting percentages and multiple-tier subcontractor use do not apply to Federal-aid Local Agency Program (FDOT LAP) or federally funded projects. The worksheet includes a legend identifying seven reason codes for potential subcontractor deletions and requires city approval and date notation for any additions or deletions to the subcontractor list.
