Provide key milestones, a process flow chart, and proposed timeline for staffing phase-in at each location to prevent interruption in services to include a transition organizational chart and proposed staffing and timeline for each position and a contingency plan for replacing personnel during the transition period. An excel workbook capturing the requirements is acceptable and will count as one (1) page. Refer to paragraph L-6.3.3. for MS Excel submission requirements.

c. Identify all actions, in clear and understandable terms, that the offeror requires the Government to perform in order for the offeror to perform its Transition.

The Government will not consider performance on an effort that concluded more than eight (8) years prior to the issuance date of this solicitation. The offeror shall include (1) a summary of the technical experience, (2) contract references corroborating recent and similar technical scope experience.

L-7.4.1.3.1. Summary. Describe roles of offerors and all subcontractors, teaming partners, and/or joint venture partners in relation to technical experience information IAW paragraph L-7.4.1.3.2.1. This summary shall include the contract number/task order number, contracting organization, and government point(s) of contact for the contract. The Government reserves the right to contact the POC to validate the technical experience submitted.

L-7.4.1.3.2. Technical Experience References (see Attachment 1). Technical Experience Information shall be submitted using Attachment 1. The Offeror shall provide a Technical Experience Contract Reference sheet for each referenced contract. The offeror shall submit a maximum of three (3) contract references corroborating recent and similar scope work as the prime contractor. If the Offeror does not have any technical experience where it performed as a prime contractor, the Government will accept technical experience from a teaming partner, mentor/protégé, subcontractor, or where the offeror performed as a major subcontractor if the portion of the work completed was similar in size/scope to this requirement. Any teaming partner arrangements, joint ventures, mentor/protégé, or subcontracting teams will be considered. Contract references are required on the offeror and any teaming partner or major subcontractor that will perform at least 20% of this effort.

L-7.4.1.3.2.1. Similar Scope Work. Offerors shall submit experience information on contracts they consider demonstrates their ability to perform the PTR OM&S requirement in accordance with (IAW) the SOW. L-7.4.1.3.2.4. Blanket Purchase Agreements (BPAs). BPA contract numbers are not allowed to be used as a technical experience reference. However, the individual call/delivery order numbers issued against the BPA are allowed. Offerors using single-award BPA call/order numbers as a technical experience reference shall include BOTH the BPA number AND the individual call/order number to verify and validate that the BPA being referenced is in fact a single award BPA. A maximum of three (3) single-award BPA call/orders issued against a single-award IDIQ can be combined and counted as one (1) technical experience reference to address the criteria. Multiple award BPA call/delivery orders issued against a multiple-award BPA cannot be combined and will count as one contract reference.

L-7.4.1.3.3. Organizational Structure Change History. Many companies have acquired, have been acquired by, or have otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of similar scope efforts or between conclusion of recent past efforts and this source selection.
