About this file
This Scope of Work Chart is a planning and evaluation template for the LBRA Produce to Market program in California, outlining the structured approach for program development and implementation across three key phases. The document provides a framework for identifying specific, measurable, achievable, relevant, and timebound (SMART) objectives across the planning, implementation, and evaluation phases. The planning phase example spans from October 1, 2022 to December 1, 2022, with the primary objective of identifying and recruiting at least five markets to participate in the program through collaboration with the Health Department, supported by documentation including outreach materials and partner forms. The implementation phase runs from January 1, 2023 to June 1, 2024, focusing on executing the Produce to Market program across ten recruited markets with responsibility assigned to the Program Coordinator. The evaluation phase concludes the cycle from July 1, 2024 to September 30, 2024, with the objective of assessing program outcomes and impacts through collected evaluation data.
The document serves as an instructional template designed for proposers to customize with their own program-specific objectives, timelines, responsible parties, and supporting documentation. No pricing terms, funding amounts, set-asides, award dates, or contract renewal options are specified within this scope of work document, as it functions as a planning instrument rather than a comprehensive contract specification.
