Use/relationship: A Request for Variance (RFV) describes a proposed departure from (a nonconformance with) approved product definition information for a limited amount of time/specified effectivity or a product found to be nonconforming with product definition information after the product has been produced by the production process.

When the RFV describes a proposed departure from approved product definition information

(i.e., pre-production), the variance shall result in a reduction in cost; indicated by enclosing the value in parentheses.

When the RFV describes a product found to be nonconforming after the product has been produced (i.e., post-production), the variance shall result in a reduction in cost; indicated by enclosing the value in parentheses.

Title: Risk Management Status Report

Use/relationship: The Contractor’s Risk Management Status Report will be used to document the contractor’s progress for: risk identification; risk mitigation planning: risk management plan implementation; and risk tracking. This data will allow management to make informed decisions.

This Data Item Description (DID) contains the format, content, and preparation instructions for the data product resulting from the work task specified in the contract.

Requirements:

1. Format. The Risk Management Report shall be in contractor’s format.

2. Content. The report shall contain a title page identifying the following:

a. Contractor’s Name

b. Contract Number and Dates of Award (including the latest modification)

c. All dates inclusive to the tasks for the reporting period.

2.1. The report shall contain the contractor’s progress in risk identification, analysis, implementation of mitigation, and tracking.

2.2. The report shall identify efforts made in assessing the current consequence of the identified performance (P), schedule (S), and cost (C) risks on the program, and shall contain a projection of the risk mitigation implementation.

2.3. The report shall contain a five (5) by five (5) matrix to identify risks to Performance (P), Schedule (S), and Cost (C). A vertical axis shall indicate the “Likelihood” and the horizontal axis shall indicate the “Consequence” of a root cause occurrence. The level of risk for each root cause shall be identified as green low, yellow for moderate, or red for high.

2.4. The report shall contain a narrative briefly describing the risk, title, type (P, S, or C), root cause, current mitigations, and planned mitigations efforts identified , along with critical dates (risk reduction milestones).

a. Group I. Definition - Data which is not otherwise essential to the contractor's performance of the primary contracted effort (production, development, testing, and administration) but which is required by DD Form 1423.

Estimated Price - Costs to be included under Group I are those applicable to preparing and assembling the data item in conformance with Government requirements, and the administration and other expenses related to reproducing and delivering such data items to the Government.

b. Group II. Definition - Data which is essential to the performance of the primary contracted effort but the contractor is required to perform additional work to conform to Government requirements with regard to depth of content, format, frequency of submittal, preparation, control, or quality of the data item.

Estimated Price - Costs to be included under Group II are those incurred over and above the cost of the essential data item without conforming to Government requirements, and the administrative and other expenses related to reproducing and delivering such data item to the Government.

c. Group III. Definition - Data which the contractor must develop for his internal use in performance of the primary contracted effort and does not require any substantial change to conform to Government requirements with regard to depth of content, format, frequency of submittal, preparation, control, and quality of the data item.

The existing Engineering Performance Specification (EPS) shall be updated to include cumulative updates from system changes.

BLK 8: The government requires thirty (30) calendars to review and/or verify the draft is in compliance with the PWS.

BLKS 10, 12 & 13: The contractor shall deliver a SCN whenever there are changes to a specification caused by the submittal of an ECP. Delivery of a SCN shall be thirty (30) calendar days after government's verification of an ECP.

The contractor shall submit the final SCN NLT fifteen (15) calendar days after receipt of the government’s verification of the draft.

BLK 14: Data to be provided on CD-ROM or other approved media as authorized by the Government.

The Contractor shall provide a completed copy of the certification required by DFARS 252.227-7017, “Identification and Assertion of Use, Release, or Disclosure Restrictions”. Furthermore, the cover page of any delivered CDRL shall contain the Special License Rights restrictive markings prescribed by DFARS 252.227-7013 and 252.227-7014 or the Government Purpose Rights restricted markings prescribed by DFARS 252.227-2013 and 252.227-7014. The Contractor shall also physically attach a copy of the applicable commercial technical data or computer software delivered as part of this CDRL.
