About this file

The document is a Schedule D-3 Cylinders Additional Items spreadsheet from NC State University, designed for a waste management contract solicitation covering the treatment, recycling, and disposal of hazardous, non-hazardous, and universal wastes. The spreadsheet lists 38 different types of cylinders and chemical containers, including gases like ammonia, argon, carbon dioxide, helium, hydrogen, and specialized chemical devices such as NanoChem OMX, Trimethyl Aluminum, and Diethyl Zinc. Each item requires bidders to provide fixed costs for four different cylinder sizes (lecture, small, medium, and large) and specify at least two treatment/disposal facilities with corresponding method codes.

The solicitation requires firm fixed costs with all fees clearly outlined, and warns that combining labor and disposal costs into a single line item will result in proposal rejection. Bidders must complete costs for every item, with $0.00 allowed only if size specifications are not available. NC State University is tax-exempt (exemption #400021), and the bidding process is conducted through the Bonfire platform. Specific instructions mandate using the provided Excel template without altering its structure, using only text-based responses, and providing detailed comments for each item. The document emphasizes strict submission guidelines, including restrictions on formatting, file saving, and response methods.

Instructions

Schedule D-3: Cylinders & Additional Items (BT-14JK)
Covers costs and services for the treatment, recycling, and disposal of hazardous, non- hazardous, and universal wastes. Firm fixed costs are required. All fees must be included and outlined in the comment section. Combining labor and disposal costs into a single line item will result in rejection of the proposal. All requirements outlined in Section 7 of the RFP apply to all cost sheets.

Facilities and their management codes are to be listed on the sheet named: Schedule D Waste Management Facilities (BT-78KM) under the supporting documents. Waste profiles are described in general in Appendix 2 of the RFP. For each profile, identify at least two treatment/disposal facilities (by reference number), the method code (e.g., H010 for metals recovery) for treatment/disposal at those facilities, and the contract cost regardless of which facility is used.

Instructions: Please provide the cost for each item or service. If indicating "$0.00" because you do not have the size, please specify that in the comments. Specify additional information in the comments section or enter NA if you do not have any comments. NC State University is exempt from sales and/or use taxes on qualifying purchases. Tax exemption # 400021.

Please Note: If you have any additional items, please fill out the Additional Responses template accordingly and provide a description in the comments box for the added item.
