About this file
This is a Qualifications and Requirements Matrix for the Wisconsin Department of Health Services' Request for Bid (RFB No. S-0627 DCTS-17) to procure vendors to perform Preadmission Screening and Resident Reviews (PASRR) Level II screens for prospective and current nursing home residents in Wisconsin. The services target individuals with serious mental illness or intellectual/developmental disabilities and must comply with federal PASRR regulations. Bidding vendors must meet mandatory qualifications including authorization to do business in Wisconsin, absence of affiliation with nursing facilities, provision of at least one state PASRR entity reference demonstrating three years of similar work, and overall qualification to meet all RFB requirements.
Vendors must demonstrate mandatory specifications including adequate day-to-day work plans, staffing structures, and qualified personnel. Key Personnel requirements include a qualified mental health professional and board-certified psychiatrist for mental illness cases, and a Qualified Intellectual Disability Professional for developmental disability cases. Vendors must also provide access to licensed physicians or physician assistants, licensed psychologists for psychometric evaluations when necessary, and sign language and foreign language interpreters as needed. All responses to the seven requirements must be marked either "Yes" or "No," with "Yes" indicating the vendor can meet the requirement using standard offerings, workarounds, or professional services at no additional cost to the State. Failure to select "Yes" for all requirements will render the bid non-responsive, though the State reserves the right to waive requirements if all vendors cannot comply.

Schedule 3–Qualifications and Requirements Matrix
Bidding Company Name
Instructions: Respond to each requirement by checking Yes or No. The only acceptable responses are Yes or No, applied by checking the applicable box. In order to be considered responsive, the Vendor must select Yes to all of the bid requirements. Selecting Yes means the requirement can and will be met with the Vendor’s current standard offering and/or using workarounds, features currently in development, or professional services to meet the need at no additional cost to the State.
Selecting No means the requirement absolutely cannot be met with the Vendor’s current standard offering and no possible developments or professional services available to the Vendor will achieve compliance. Cells without a Yes or No responses selected will be interpreted as No.
If it is determined that all Vendors submitting a bid cannot meet a particular requirement, the State reserves the right to waive that requirement and continue with the solicitation process.
