About this file

This document is a sample notice of entry related to a traffic disposition forms fabrication, printing, and delivery solicitation issued by the District Court of the First Circuit of the Judiciary, State of Hawaii. The solicitation requests quotes from vendors to fabricate, print, and deliver 120,000 sets of Notice of Entry of Judgment and/or Order forms, commonly referred to as Traffic Court or Traffic Disposition forms. Contractors must produce forms identical to the provided samples in all aspects except for specified wording changes noted in the accompanying PDF file. The sample forms control over written specifications in cases of discrepancy. The contractor is responsible for delivering the exact quantity specified with no overages or shortages permitted. All deliveries must be made to the designated location and include freight delivery to floor, with materials stacked in proper drawing sequence for easy retrieval. The Judiciary will not compensate for any overages delivered by the contractor.

All costs for labor, equipment, materials, transportation, and other necessary appurtenances are the contractor's responsibility. Should the Judiciary require an increase in total quantity prior to contract runoff, the unit price for additional sets must be equal to or less than the quoted price. Detailed specifications are available in the separate SPECIFICATIONS Traffic Disposition document. Contractors may request inspection of sample forms upon inquiry.
