VA Boston Healthcare System West Roxbury VA Medical Center 1400 VFW Parkway West Roxbury, MA 02132
STATEMENT OF WORK
523A4-24-010 Remove and Replace Pool Roof
1.0 INTRODUCTION
This project primarily includes the replacement of the Pool Roof and the roof on building 2 above the Patient Ward approximately 2,000 SF on the connection between buildings 1 and 2. Work includes the following: Contractor to provide all equipment, labor, and materials for a complete roof replacement. Work is not limited to but includes the following:
· Labor material & equipment to remove existing solar thermal heating systems on the rooftop.
· Reinstall the solar panel support system & remove panels offsite.
· Labor, materials, and equipment necessary to provide removal of existing rooftop equipment -disconnect/reconnection and/or relocation of other rooftop equipment and associated ductwork/piping to accommodate complete roof replacement.
· Labor and materials for new roof replacement.
· Provide 30–year manufacturer’s warranty.
· GC to provide shop elevations/drawings to ensure proper drainage.
· Work shall be performed at VAMC West Roxbury in accordance with the Statement of Work, Specifications, Drawings, Federal, State, and Local Codes.
The period of performance for the above-mentioned project will begin with a Notice to Proceed (NTP) issued at a Pre-Construction Meeting (Pre-Con) and shall not exceed 180 days after the NTP. This period of performance includes 15 days for all submittals to be approved prior to beginning construction. As part of this solicitation, the General Contractor (GC) shall submit an electronic PDF preliminary schedule showing the methodology for accomplishing the project within the period of performance.
2.0 GENERAL REQUIREMENTS
1. Contractor’s employees shall not enter the VA Medical Center site or project site without an appropriate VA-issued PIV badge. See specification 01 00 00 subpar 1.5 Construction Security Requirements for more full requirements.
2. Work hours are 6 am-230pm Monday through Friday, except for the following: a. demolition of existing roof and removal of roof debris weekends or holidays; b. delivery of roof material after 6 p.m. on weeknights, weekends, or holidays.
3. The dumpster, crane, and delivery staging area is the building 2 loading dock driveway.
4. The Contractor shall furnish & post a board where directed by the Contracting Officer, VA Project Engineer (COR), and VA Safety Specialist that includes, but is not limited to, a copy of Rates and Wages, the Davis-Bacon poster & and schedules, ILSM/ICRA/GEMS/Patient Safety signed determinations, Site Specific Emergency Contact information, Construction site fire response procedures & evacuation plan, post the number of accident free days, emergency force contact information with nearest hospital including directions, etc.
5. All utility tie-ins and interruptions of utility systems require prior approval and scheduling with the VA Project Engineer (COR). The contractor shall submit a request to interrupt any utility systems to the VA Project Engineer (COR) in writing at a minimum of three (3) weeks in advance of the proposed interruption. See specification 01 00 00 subpar 1.6 Operations and Storage Areas for full requirements.
6. The Contractor shall not proceed with mobilization or construction until after Veteran Affairs accept all required submittals and coordination drawings.
7. The Contractor shall follow the VHA BHS West Roxbury Project’s submittal process attached to specification section 01 00 00.
8. The contractor shall submit a Site Superintendent Designation Letter designating the Site Superintendent to be dedicated to this project only. The Site Superintendent shall be always physically present at the construction site (West Roxbury) if there are contractors’ employees or suppliers present, work activities are ongoing, materials are being delivered, etc. The contractor shall submit, via e-mail, a list identifying key personnel, their responsibilities, contact info, and 24 hour/7 day per week primary and secondary contacts. Superintendent Communications: At all times during the performance of this contract, the designated Site Superintendent is to be available by cell phone. After-hours phone number shall be posted at the construction site on the required board in item #2 above.
9. Before placement and installation of work subject to acceptance checks and tests, the contractor shall coordinate and notify the VA Project Engineer (COR) in sufficient time to enable testing personnel to be present at the site in time for proper testing and field inspection. This includes notification prior to closing walls, ceilings, chases, etc. Such prior notice shall be not less than two (2) weeks unless otherwise designated by the VA Project Engineer (COR).
3.0 SAFETY PRECAUTIONS
1. The Contractor shall comply with all applicable Federal, State, and local legal requirements regarding worker’s health and safety. The requirements include but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR) Parts 1910 and 1926. The contractor is solely responsible for determining the legal requirements that apply to activities and shall ensure safe and healthful working conditions for its employees.
2. Contractor shall assume the responsibility to guard against causing fires and/or explosions and to protect Government Property.
3. The Contractor shall perform the work in a manner consistent with the area security, patient safety and fire safety regulations, especially regarding exits, exit way access, and roof access.
4. Utility shutdowns shall not compromise security, communication, HVAC, or fire safety for occupants.
5. No flammable liquids shall be stored or used in the medical center or on the roof.
6. The required number and appropriate types of portable fire extinguishers are required per National Fire Protection Agency (NFPA) 10 and NFPA 241. The contractor shall keep certification on-site at all times of extinguisher inspections.
7. The Contractor shall receive from the COR a permit for all cutting, welding, and soldering 24 hours in advance. All permits shall be prominently displayed during all construction.
8. All necessary precautions shall be taken by the contractor to prevent accidental operation of any existing smoke detectors or sprinkler heads.
9. Present the contractor’s fall protection and safety plan to the COR, engineering manager and safety specialist prior to the start of work.
10. The Contractor shall comply with an Infection Control Risk Assessment (ICRA) which will be developed with the COR and the Infection Control Practitioner assigned to the project at the Preconstruction Conference. Multiple ICRA's may be necessary to address specific risks at various stages of the project and must be approved prior to proceeding on each phase. The Contractor shall also comply with Interim Life Safety Measures (ILSM) & Green Environmental Management System (GEMS) which will be developed with the COR.
4.0 HAZARDOUS MATERIAL REPORTING
1. The VA BHS asbestos survey reports negative findings for asbestos in the work area.
2. The Contractor shall maintain hazardous material inventories and material safety data sheets (MSDS) for all hazardous materials (as defined in CFR 1910.120, 40 CFR's 355, 370, & 372) to be stored and used on this Medical Center. Hazardous materials must be inventoried when received and at the project's completion. The amounts used shall be maintained for the project duration, and for the calendar year (ending 31 December).
3. Hazardous Materials Inventories, Material Safety Data Sheets and material quantities used shall be submitted to the Contracting Officer for approval.
4. In the event of a spill, the Contractor shall immediately notify the Contracting Officer's Representative (cell 617-390-4694) and GEMS specialist (cell 617-272-6229) as well as the Contracting Officer. The Contractor shall be solely responsible for the expense of any cleanup of such spill, and the cleanup shall be in accordance with the applicable provisions of 40 CFR Part 761.
5.0 ENVIRONMENTAL PROTECTION
1. To provide for abatement and control of all environmentally hazardous materials arising from demolition and/or construction activities, the Contractor shall comply with all applicable environmentally hazardous material control and abatement and all applicable provisions of the Corps of Engineers’ Manual EM 385-1-1, “General Safety Requirements” as well as the specific requirements stated elsewhere in the Contract Documents.
2. The contractor is responsible for daily cleanup of all areas affected by construction. Construction areas in use or affected shall be returned to the condition in which they were turned over or initially found. VA Housekeeping shall not be dispatched for cleaning associated with contractor construction.
3. The contractor shall take every precaution in preserving flooring, finishes, equipment, and furniture in areas of construction. The contractor shall repair or replace any damage incurred during construction at their expense.
6.0 SECUIRTY REQUIREMENT
1. All contractor personnel shall obtain a short-term identification badge issued by the COR or government-designated representative. Such badge shall be worn by the individual and prominently always displayed while on VA Property. No employee of the contractor shall enter the project site without a valid identification badge issued by the VA. To obtain a short-term identification badge, contractor personnel shall present to the COR a valid (non-expired) photo identification issued by a US federal, state, or local government agency.
2. The Contractor shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in standard acquisition guidelines. Per the VA handbook 6500.6, no VA-sensitive information will be transferred, shared, or stored with said vendor. As such, no additional mechanisms will be needed to ensure the protection of information.
