About this file
This document contains a Statement of Special Inspections and Schedule of Special Inspections for facility renovations, improvements, and repairs at the Long Island National Cemetery in Farmingdale, New York. Special inspections will be required for structural steel, steel construction other than structural steel, concrete construction, masonry construction at Levels B and C, wood construction, soils construction, driven deep foundations, cast-in-place deep foundations, helical pile foundations, sprayed fire-resistant materials, mastic and intumescent fire-resistant coatings, exterior insulation and finish systems, fire-resistant penetrations and joints, smoke control, architectural components, and mechanical and electrical components. The related federal contract opportunity notice replaces solicitation 36C786-20-B-0011 with 36C786-20-B-0029, which is now open to all small business concerns rather than just service-disabled veteran-owned small businesses, as the rule of two criteria for a total set-aside could not be met under the original solicitation. The Department of Veterans Affairs National Cemetery Administration is the contracting agency.

☐ The Contractor’s Quality Control (QC) Manager ☒ The Special Inspector of Record (SIOR) shall oversee the special inspection requirements defined in this statement. The qualification requirements and responsibilities for the Special Inspector of Record (if required), Contractor’s Quality Control (QC) Manager, and various inspection and testing agents, report submittal requirements, discrepancy tracking procedures, and other procedures necessary to execute the special inspections program are defined in project specification 01 45 35.00 SPECIAL INSPECTIONS.
The Special Inspection program does not relieve the Contractor of the responsibility to comply with the Contract Documents. Jobsite safety and means and methods of construction are solely the responsibility of the Contractor.
This Statement of Special Inspections includes a Schedule of Special Inspections attached herein that defines the following:
• Materials, systems, components and work required to have special inspection or testing
• Type and extent of each special inspection
• Type and extent of each test
• Additional requirements for special inspection or testing for wind or seismic resistance where required
• Identification as to whether special inspection is continuous or periodic for each type of special inspection
The Schedule of Special Inspections identifies the following elements of the buildings and structures for this project that require verification and inspection in accordance with IBC 2012 Section 1705.
