About this file

This document is a Standard Operating Procedure (SOP) for the Dig Permit Program at the Edward Hines, VA Hospital in Illinois. The SOP defines the process for obtaining approval to excavate or disturb underground utilities and infrastructure within the hospital's area of responsibility. It outlines the responsibilities of key personnel, including the Engineering Service Chief, Project Planning Chief, Dig Permit Program Manager, and Execution Agent. The SOP details the required steps, such as requesting a dig permit, locating utilities, completing the Hines Form 1532, and obtaining approvals prior to commencing any excavation work. The purpose is to minimize the risk of damage to critical underground infrastructure during construction activities. The SOP references relevant OSHA and USACE safety requirements. This document does not appear to be directly related to the provided federal contract opportunity.

This form is used to coordinate the required work with key installation activities and keep customer inconvenience to a minimum. It is also used to identify hazardous work conditions in an attempt to prevent accidents. The Dig Permit request is processed just prior to starting work. If delays are encountered and the conditions at the job site change (or may have changed) this permit must be reprocessed.

16. REMARKS (This section must describe the specific measures to be taken before and during work accomplishment. Specific comments concerning the approved method of excavation, hand or powered equipment, must be included.)
