About this file
This document is a Bid Cover Sheet (Attachment B) for Invitation to Bid (ITB) S-10000-00012052 issued by the State of Oregon's Department of Administrative Services (DAS) Procurement Services for Mobile Office Van Modifications. The document is Version 1.0, dated February 20, 2019, and serves as a mandatory form that potential bidders must complete and submit with their bid proposal. The cover sheet requires bidders to certify various statements about their business practices, tax compliance, non-discrimination policies, and legal standing.
The bid cover sheet includes critical certifications for bidders, such as confirming they are not on federal exclusion lists, have no conflicts of interest, have prepared their bid independently, and comply with state tax laws. Notably, for contracts anticipated to be $150,000 or more, bidders must have a written policy preventing sexual harassment, sexual assault, and discrimination. The document also highlights that false statements or misrepresentations could result in potential legal consequences under the Oregon False Claims Act. Bidders must provide company details including name, address, contact information, state of incorporation, and business registry number when completing this form.
